Employee was operating CAT 775D haul truck in the pit. Employee lost control of the haul truck and turned the truck into a side ramp resulting in the haul truck turning over on its side. Employee was wearing seat belt at time of incident.

While grabbing onto a metal screen to remove it from the forks of the lull the screen shifted and its edge cut through gloves and caused a laceration into the palm of right hand requiring 4 stitches.

Injured employee was assisting with changing shim plates in the primary jaw crusher. As EE lifted the shim plate EE strained back. EE was taken for medical evaluation where EE was diagnosed with a thoracic strain and placed on restricted duty.

Employee was working on the #1 conveyor belt at the big jaw crusher that was choked down with material. Upon completion, employee noticed more rock underneath of the belt. As employee reached to remove the rock, the belt started, causing employee to jump clear of the belt and upon landing, right ankle rolled over causing injury to right ankle.

While driving haul truck across rutted section, truck bounced in a manner that caused injury to lower back. Medical treatment resulted in time off/modified work duties.

Hauling shot rock to Jaw (primary) - while returning to the truck from a restroom break, rolled right ankle while stepping on a rock, didn't see. Medical treatment resulted in modified work duties. Return to work pending follow-up with doctor.

While shoveling at the secondary plant, the employees foot slipped from the shovel and when employees foot landed, employee felt a sharp pain in left lower back & buttocks area. Medical attention resulted in restricted work activity.

While replacing and tightening a bolt on a plant employee felt/heard a pop in right shoulder. After tightening a bolt on a plant, employee started to feel pain and had difficulty moving arm. Medical attention resulted in restricted work activity.

Over Christmas weekend, between 12/23 and 12/27/2016, refuse pile gave way causing damage to drag line, box ben and #2 conveyor. No injuries occurred.

While employee was doing clean-up work using a shovel, ee reported ee injured back. Medical attention at local hospital.

While walking to the Stedman feed belt at secondary plant, EE slipped & fell at a muddy spot hitting right arm on a rock that had been removed from the feed belt. According to foreman, no trip hazard, just mud from all the rain. EE take to ER then transferred via ambulance to Hospital where her right elbow was reset. EE remains off work pending further evaluation

While climbing down from the cab of a bulldozer, placed right foot on the step down bar, then lifted his left foot to step down onto ground when his right foot slipped between the step down and the tracks. He fell backward with his right leg caught straining right knee. Treated at ER, off work until further evaluation.

While replacing a wall hammer in the crusher the hammer slipped out of position. Injured person & another employee lowered the hammer to re-position their grip so the hammer did not fall onto a 3rd employee. IP did not remove his hand causing the hammer to sit down on his hand pinching his hand/fingers. X-rays revealed no fractures & he was released to return to work.

While turning hammers over in the Stedman crusher and being assisted by another EE the other EE's end slipped from his hand crushing injured employee left ring finger. His finger was caught between hammer and the plated it attaches to causing laceration and contusion. First aid was performed at plant. ER treated and released day of injury. X-ray at ER showed not broken bones.

While lacing the lime screen belt back together with electric drill, the drill twisted causing his hand to be torqued before he could let go of the drill. Thought was a sprain, but next morning hand & finger swollen. Medical treatment 7/16/15 revealed broken right ring finger. Company accommodating restricted work activity.

While replacing the skirt rubber at pioneer #3, noticed the hole was not correct and as employee was re-cutting the hole, his knife blade slipped cutting the top joint of his left thumb. First aide performed at facility. ER treated and released day of injury.

While backing CAT truck into position to be loaded, employee failed to notice the loader bucket was not high enough thus causing contact between both CAT truck & loader while still in backup motion. Resulting in a whiplash type motion causing pain in his neck and a headache. Minor medical treatment on 11/13/14.

Employee was holding jaw bolt & pin while the pin was being hammered out; thought his hand was clear of pinch point but as the pin came free, his hand slipped behind pin & wall pinching his right ring finger. Medical attention on 9/3/14, Restricted work activity.

Employee finished grinding metal, while removing safety glasses, felt something enter his right eye. He used wash solution at the 1st aid station and thought ok. Overnight his eye became irritated, sought medical attention on 4/8/14. Doctor treated and released.

While employee was tightening up jaw die bolts at big jaw crusher #521013; his feet slipped on some grease causing him to over extend himself with his right leg; resulting in injury to groin area. Medical treatment on 6/29/12; modified work for 3 days

EE was on the drive side on the tunnel conveyor hooking up a comealong to pick up bottom of the belt off old wooden crosstie. New wooden c.t.'s being pushed in place across the metal frame to the drive side of conv., the EE reached down to grab c.t. and mashed finger tip between wood and conveyor metal. ER 2/22/12 Surgery 2/23/12, Rt. middle finger tip. Remain off work.

EE was on the discharge belt to get a wedge off of it, when the plant operator did not see him and started up the belt. EE's left hand got caught between the roller and conveyor belt. EE was treated and released from the ER with bruising.

While repairing a conveyor belt, EE was cutting a hole in the belt using a utility knife; the knife slipped and severely cut EE's left hand. EE returned to restricted work activity on 7/29/10.

EE was struck on the right shoulder with a stick by another employee who was mad at the S.N.P. causing him bodily harm. (X-ray & treatment) Per foreman the ER Dr stated it was knocked out of socket & he popped it back in. Referred to family Dr if further treatment needed.

Employee was shoveling in secondary plant. Shovel struck against a metal support leg jarring her right hand and wrist. Employee sought medical attention (7-6-05) at which time she was prescribed medication and placed on light duty for one week.

EMPLOYEE WAS WORKING ON THE ROBBINS DRILL HELPING TO HANG A PIECE OF DRILL STEEL PER INTERPRETOR HE JUMPED DOWN WHILE HOLDING ON TO RAIL & HURT RT. SHOULDER. 6/18/05 WENT TO ER & WAS TOLD RT. SHOULDER WAS DISLOCATED, REPORTED TO ANOTHER FACILITY 6/19/05 DUE TO AFTER SHOWERING HIS RT. SHOULDER CAME OUT OF PLACE, SURGERY PERFORMED ON 6/20/05.

C-CLAMP WAS PUT ON PIECE OF METAL ON WASHING SCREEN TOWER, WHEN PIECE OF METAL WAS HIT WITH A HAMMER THE C-CLAMP BROKE HITTING EE IN THE LEFT KNEE CAP. EE SOUGHT MEDICAL ATTENTION DATE OF INJURY, RETURNED TO WORK, NO RESTRICTIONS. EE RETURNED TO PHYSICIAN ON 01/20/05. EE TO REMAIN OFF WORK UNTIL MRI RESULTS RECIEVED.

WORKING IN SECONDARY PLANT, WRENCHED BACK. DID NOT WANT TO GO TO THE DOCTOR AT THIS TIME. STATED HE WOULD SEEK CHIROPRACTIC CARE AT HIS EXPENSE. (HE WAS USING SHOVEL) FIRST REPORT OF INJURY WRITTEN OCT 1, 2004 FOR RECORD ONLY. MEDICAL ATTENTION SOUGHT ON OCT 1, 2004. ACCORDING TO WORK COMP INS CARRIER ON APRIL 8, 2005 - RELEASED NO RESTRICTIONS 2/16/05.

EE WAS GRINDING ON FRAME WORK IN SIMPLICITY WASHER SCREEN IN THE SECONDARY PLANT WHEN A PIECE OF METAL BOUNCED OFF SIDE WALL AND WENT INTO LEFT EYE (EE WAS WEARING SAFETY GLASSES AT TIME OF AC CIDENT).

EMPLOYEE WAS TAKING OFF BOLT AND NUT ON SKIRT BOARD ON CONVEYER WHEN THE METAL BACKBOARD SLIPPED DOWN AND CAUGHT RIGHT FINGER BETWEEN METAL AND WRENCH.

USING SHOVEL TO CLEAN OUT FALLING DIRT UNDER CONVEYER, SECONDARY PLANT, TURNED WRONG WAY AND TWISTED BACK.

WRENCH SLIPPED OFF NUT AND EMPLOYEE SLIPPED ON GUARD AND STRADDLED IT, LANDING ON HIS LEFT GROIN AREA - WAS WORKING ON WASHING SCREEN. NO MEDICAL ATTENTION NEEDED AT TIME OF INJURY - NOTIFIED BY SUPERINTENDENT OF MEDICAL ATTENTION RECEIVED TODAY (10-22-02).

EMPLOYEE LOOSENING BOLT ON 300 HP MOTOR AT OLD JAW #521004 AND MOTOR SLID FORWARD AND RACHET PINNED EMPLOYEE BETWEEN THE FRAME AND MOTOR ABRASION OF RIGHT FIBULA.

EE HAD RIGHT HAND ON OUTSIDE OF BOBCAT MACHINE; WAS BACKING UP EQUIPMENT WHEN SHE RAN PAST A METAL SUPPORT BEAM ON PIONEER SCREEN AND SMASHED HER TWO MIDDLE FINGERS ON RIGHT HAND. EE CURRENTLY ON LIGHT DUTY ASSIGNMENT AS OF 8-8-00.

EMPLOYEE WAS CUTTING A METAL PLATE AT PRIMARY CRUSHER WHEN FIRE FLEW UP OVER HIS WELDER HELMET &LANDED IN HIS RIGHT EYE.

EE WAS PICKING UP A 150 LB HAMMER FOR THE STEADMAN CRUSHER (522029) AND STRAINED HIMSELF. EE NOTIFIED EMPLOYER OF INJURY - JUNE 10, 2000.

EE GOT UP ON TAILWHEEL OF SIMPLICITY SCREEN CONVEYOR COVER PLATE WHILE SPRAYING BELT DRESSING ON THE TUNNEL BELT AT THE HEADWHEEL. HIS HAND GOT CAUGHT AND WAS PULLED AROUND THE HEADWHEEL SECTION

WRENCH CAME OFF OIL FILTER HTTING EE STITCHES REQUIRED.

TRUCK WAS JARRED WHILE BEING LOADED. EE WAS LOOKING UP AND DIRT FROM HEADLINER FELL INTO EYES.

EMPLOYEE FELL WHEN BELT ON CONVEYOR STARTED UP WHILE HE WAS ON IT.

TURNED ANKLE WHILE GETTING OFF A PIECE OF EQUIPMENT.

EE WAS CLEANING A HIGH PRESSURE PAINT GUN TRIGGER WAS DEPRESSED AND A MIXTURE OF AIR AND MINERAL SPIRITS WAS BLOWN INTO MIDDLE FINGER THRU FINGER NAIL.

LIFTING AND PUTTING THEM IN THE PIONEER SCREEN,S LIPPED AND FELL ON BOTTOM DISCHARGE BELT WRENCHING BACK.

AUGER TUBE ON TRUCK SWUNG AROUND HITTING EMPLOYEE IN CHEST.

FLASH BURNED EYES WHILE ASSISTING A WELDER.

EMPLOYEE WAS TAKING GUARD OFF SCREEN WHEN THE BELT MOVED BACKWARDS, PINNING HIS ANKLE TO THE CONVEYOR. HE INITIALLY DID NOT MISS ANY WORK DAYS, BUT INFECTION SET IN CAUSING HIM TO BE OFF AT A LATER DATE.
