The injured employee was reconnecting a 8 inch aluminum vacuum pipe that had been clogged. When connecting the pipe the stored energy cause by the 6 inch flex attached to the 8 inch pipe spun out of the employee's hand. When the pipe left EE's hand it cut EE's right middle finger.

EE was performing maintenance when left thumb was caught between a metal screen cloth and a handrail, sustaining a laceration.

Employee had right forearm mashed between two pieces of metal during plant maintenance causing a contusion.
Employee experienced back strain while assisting two other employees change screens.
While breaking the studs off of a belt fastener the worker sustained a laceration to the top part of wrist requiring 4 stitches.

EE entered the bed of a pickup truck to retrieve a tool. The tailgate was in the up position and after retrieving the tool, the EE attempted to exit the pickup truck facing forward and caught trailing foot on the top of the tailgate and fell forward injuring their knee. EE was taken for evaluation and returned to work. On 4/13/21 orthopedist ordered EE out of work.
EE entered the primary hopper to assist with dislodging a rock hung up in the grizzly bars at the jaw crusher. While EE's left foot was firmly planted and wedged into a tight location EE lost footing with EE's right foot, causing EE's left knee to twist. The employee was taken to a local clinic for evaluation where it was diagnosed as a strained MCL and torn meniscus ligament.

Employee was operating an off road dump truck while traveling down the ramp towards the pit. The truck gained speed and the driver maneuvered the truck towards the berm. Once the truck made contact with the berm, the truck flipped onto its side.

While attempting to puncture a 1-1/2" rubber hose with EE's non-locking pocket knife, the EE had the blade close on index finger causing a laceration across the first knuckle above the finger nail of gloved right hand. At the emergency room 5 stitches were needed to close the wound.

EE EXITED CAB AND WALKED ACROSS TOP OF RADIATOR TO CLEAN RIGHT SIDE VIEW MIRROR. WHILE RETURNING TO CAB EE AGAIN WALKED TOP OF RADIATOR AND FELL APPROXIMATELY 8 FEET TO THE GROUND BREAKING LEFT LEG.
EE was attempting to access the cab of the drill when they stepped onto the rear section of the track and then onto the first step, when their muddy right boot slipped across the surface. When catching themselves and trying to pull themselves up into the cab they strained their back.
While welding on the Stedman crusher, EE had a piece of hot slag hit their face under the welding hood and safety glasses. EE removed their welding gloves and rubbed their face where the slag had hit and brushed particles of metal into the right eye.

EE was helping change hammers in the Stedman. EE had a pry bar wedging one hammer up to assist tow other EE to install the hammer into place. Hammer went into place but caused ee hand to be pinched between bar & hammer.
Hearing loss in left ear per copies of the report which indicated the injury for 3 different audiometric tests done on 2/10/15 and 3/16/16.

Injured employee was helping another employee get a screen loose on the pep screen. They pulled on the screen and when it came loose, injured employee's right hand was mashed between the lever and the pep screen. Injured employee remains off work pending further doctor evaluation.
While cleaning the front window on CAT 769c truck, stepped back and foot slipped, caught right forearm on the hand rail. Treated/Released from clinic.

While hauling a load of rock off the mountain to the crusher; employee stated the truck started bouncing in the front end. The truck hit two boulders, knocking them off the high wall. Impact caused the steering wheel to jerk the employee's left hand resulting in injury to wrist on the left side. Medical treatment, restricted work activity.
While working in the air separator, operating a hand grinder to grind down the welds on the sides and wearing safety goggles and a full face shield, employee got something in his right eye. Medical Treatment on 3/7/13 with follow-up scheduled.

Employee was jacking trailer off the ball-hitch was stuck on ball-when hitch release it smashed right index finger between the hitch on the trailer and the military hitch on truck. He worked 5/2/08 and is currently off work, effective 5/5/08. Note: currently employee tn shop-delivery to huntsville al quarry.

Employee was cleaning out a chute on the air separator. Went to get foreman to show him something, was walking back and stepped on uneven ground causing right knee to twist and he fell to his hands and knees. Currently working under restricted work activity.
TN welding shop crew working at Huntsville Quarry for modification to plant. Employee was working inside air classifier, got out and came down ladder, caught left elbow on edge of cage surrounding ladder resulting in miner cut that he cleaned & applied bandaid to. Wed. 8/15/07 elbow hurting & seeing swelling, sought medical attention 8/17/07. Follow-up 8/24/07.
Employee was operating dragline, cleaning out settling pons. Pulled friction bars back and released brakes. The brake levers came up and injured both legs. fractured right leg. currently off work. Note: currently employee at allsboro quarry. Working at huntsville quarry day of injury.
EE was standing on side of dozer cleaning the window. He slipped & fell backwards. He landed flat on his back on the ground.

Employee was tightening belts on Stedman. Rolled belts to make sure they were tight. Belt caught glove and pulled fingers between belts and pulley. Mashed and cut middle and ring fingers on left hand and broke pinky finger on left hand. Currently working under restricted work activity.
EE parked truck, got out of the truck and the door came back on right hand mashing middle finger & ring finger. Currently working under restricted work activity.
Employee was shoveling under conveyor, rock fell and hit left hand. 1/17/07 restricted work. 1/22/07 released to regular work.

Employee was putting transmission oil in dozer. While getting down from dozer his foot slipped and he fell to the ground onto rock pile. He used his right arm to catch himself. Broke right arm near wrist. No lost time as of today but is currently on restricted work.
Employee was using punch & sledge hammer to punch holes into conveyor belt. Accidently missed punch when swinging sledge hammer & hit his thumb instead. Resulted in a dislocation to his left thumb. No lost time as of today but is currently on restricted work.
Employee backed under #9 bin to load his truck. He got out of the cab and reached the control to open bin door. He lost his balance and fell from the truck resulting in small gash to right forearm, multiple scratches to right arm, loss of skin to right shoulder, across back.
EE was using a sledge hammer to beat caps out of crusher. A foregin material from either caps or sledge hammer entered EE's right hand between thumb and index finger. He was taken to Hospital and was referred to hand specialist. Work duty is modified. Surgery scheduled for 4/20/06.

EE was on top of loader bucket doing his mid-day greasing of bucket cylinders. A shot was set off; he turned to see where rocks were going. He lost his balance and fell to the ground, bruising his left knee. He returned to work the same day (after seeing physician) and worked the next day, leaving early due to pain. He reported to the ER over the weekend for lower back pain.
Employee was leaving dispatch office (bank errand). She stepped off bottom step, shoe slipped off step and fell twisting her left ankle. She saught medical attention the next day. X-rays and medication received.
While working in crusher area, EE states he was coming down the hill, ground was wet. Truck he was driving slowly started turning sideways & next thing he knew he flipped over. His head went thru side window & armrest hit his ribs. Didn't seek medical attention immediately but called Supt later that evening for info on where to go. He was seen at Huntsville ER (2 cracked ribs).

EE WAS GETTING DOWN FROM CAT TRUCK BY USING LADDER ON FRONT OF TRUCK. FOOT AND HAND SLIPPED FROM LADDER - EE GRABBED WITH OTHER HAND TO GET BALANCE. EE FELL AND WAS SUSPENDED BY ARM AND HAND THAT GOT CUAGHT IN LADDER RUNG.
Employee was cleaning off build up that had accumulated on grease line, the ladder he was standing on shifted causing him to lose his balance. He fell approx three feet hitting his head and was knocked unconscious and injured right ankle.
EMPLOYEE WAS GREASING WATER TRUCK WITH GREASE GUN, GREASE FELL INTO HIS LEFT EYE. EE DID NOT SEEK MEDICAL ATTN. UNTIL 9/7/04, AT THE EYE CENTER, WHERE FOREIGN MATERIAL WAS REMOVED FROM HIS LEFT EYE.
Cleaning material from base of teledyne rock breaker, employee fell through hole cut in cat-walk made for repairs. He cut lower right leg, requiring stitches.

THE INJURED IS EMPLOYED BY THOMPSON MACHINERY BUT WAS WORKING ON OUR EQUIPMENT AT OUR HUNTSVILLE QUARRY WHEN HE HE WAS INJURED.AIR HOSE BECAME OFF OF EQUIPMENT THAT THE INJURED WAS WORKING ON HUITTING HIM BELOW RIGHT EYE.
