Lago Verde Mine, LLC · Caught in, under or between a moving and a stationary object

Employee climbed on belt to clear a clog, EE was not tied off, and EE did not LO/TO. The plant operator started the plant while employee was still on belt. Employee grabbed the drive pully to catch themselves and the belt cut first and second finger. Second finger was repaired in surgery. The first finger was removed from fingernail down during surgery.

Outlaw Ridge Inc · Struck against a moving object

Site inspection on 4/27/18 showed 3" of slightly loose soil conditions under the left track. The right track had solid ground conditions. The excavator was removed from the lake on 4/28/18. Inspection of the excavator showed the bucket to be 3/4 full of shot rock and curled almost up beneath and between the tracks. It appears the operator undermined self.
