Georgia Stone Products LLC · Fall to the walkway or working surface

EE was working at the plant on a clog in a discharge pipe. An other EE open the grating to clear out the discharge line. Grating to manhole was not replaced correctly and the other EE step on grating which cause EE to fall and fracture left wrist.

The miner was replacing the turn table on SCI's VSI crusher. When pushing the heavy table into place, EE sprained EE's knee.

SCI Materials, Inc. · Accident type, without injuries

SCI Materials's mechanic was preparing to jack up a stacking conveyor using a handheld 20 ton bottle jack. A vehicle was parked underneath the stacker. As the conveyor was jacked up it slowly tilted forward. The support beam of the stacker gently rested onto the hood of the vehicle, causing minimal damage to the hood and windshield. No injuries occurred.

SCI Materials, Inc. · Struck against a moving object

At 10 am on Monday, June 5th a Cat 773B 50 ton haul truck was backed up to a Linkbelt 350 excavator. While being loaded, an air valve gave way on the truck. This caused the brakes to go out, and the driver backed into the Excavator. The haul truck and driver suffered no damage/injury. The excavator and operator suffered minor damage/minor injury.
