Employee was changing light bulb. Reaching to change bulb and got off balance and fell forward onto gravel. Skinned his arms and hurt his right shoulder when he fell.

WAS GETTING A ROCK UNHUNG IN JAW CRUSHER WITH A WEDGE. THE CABLE THAT HE WAS PULLING ON HAD A FRAY IN IT AND IT PUNCTURED HIS POINTER FINGER ON HIS LEFT HAND. HE HAD GLOVES ON AT THE TIME.
