Struck by flying object

A Dragline Operator suffered a corneal abrasion to his left eye while cutting a keeper off the dragline bucket with a torch. As the injured started cutting with an oxygen/acetylene torch dirt or steel popped causing debris to enter his left eye behind his safety glasses.

Accident type, without injuries

Excessive rainfall over the past several days has caused the Kickapoo Creek to exceed its banks allowing water to collect in the pit such that normal operations have been stopped. All equipment and personnel had been removed from the pit and were not affected by the water.

Struck against a moving object

At the end of the shift, the injured was driving his personal vehicle to the locker room to pick up his dirty clothing and lunch box, when he struck the yard gate at the front of the property.

Due to heavy rains last week causing the rivers to overflow their banks, this quarry is flooded. Once waters recede, the quarry will resume operations int he next several days.

THE INJURED WAS HOLDING A STEEL LINER PLATE WHILE THE SECOND EMPLOYEE WAS STRIKING IT WITH A SLEDGEHAMMER. THE STEEL LINER SWUNG LOOSE MOVING THE INJURED'S HAND INTO THE LINE OF THE SLEDGE HAM MER. THE SECOND EMPLOYEE THEN STRUCK THE INJURED'S HAND FRACTURING HIS LEFT RING FINGER.

EE WAS SHUTTING PUMP DOWN. THE PUMP HAD CAVITATED CAUSING IT TO BECOME HOT. A 2 INCH VALVE FAILED SPRAYING EE WITH HOT WATER.
