Struck against stationary object

During installation of new screens in the screening plant, one screen did not set in properly. The employee was adjusting the screen and cut EE's finger on the new screen edge causing a laceration. The laceration required 3 stitches.

Over-exertion in pulling or pushing objects

While connecting discharge water pipeline together employee pulled the collar from the bottom of the pipe around to secure the lock straining shoulder.

With supt removing flywheel off crusher, spacer pins buckled pinching employee's hand. Employee refused treatment; had full mobility of finger & hand. 6-2-15 Employee reported everything was fine just sore & bruising. 6-9-15 employee complained of lack of full extension of pinky finger went to dr for eval & sent to specialist 6-10-15 specialist determined fracture & put in cast

While lifting the tongue on a dolly so it could be chained up, the tongue was let down and the employees 3rd and 4th fingers on his left hand were smashed. The 4th finger required stitches.

Contact with electrical current

While attempting to isolate an electrical problem, the supervisor of the Portable Plant opened portable switch box and put his right hand down in the area of the electrical breakers (without de-energizing the circuits) resulting in an electrical shock and burns. 2nd & 3rd degree burns to the right hand just below the thumb joint.

WHILE USING A GRINDING AT THE CONSTRUCTION SITE OF OUR NEW CRUSHER PLANT. A METAL FLAKE ENTERED HIS LEFT EYE.
