Fall from machine

Driving a Euclid, the top of the engine caught fire. The operator stopped the truck and exited the cab. The employee started down the access ladder and decided to jump to the ground. Approximate height 78"" to 91 1/2"".

Rotating rotor on red mill crusher to check clearance. Employee got their finger caught between the drive belts and sheave.

EE had been in tunnel putting a coil on centron. The job was finished and he was on a ladder climbing out, when a rock fell from the surge pile and mashed his hand against the ladder.

Employee was backing up in his Euclid to dump DGA in stockpile. He backed up too far and backed over the edge of the stockpile and flipped over backwards, with the Euclid landing on its wheels.

Employee was taking a pulley off shaft on screen. He was using a sledge hammer to loosen it and hit his finger.

EMPLOYEE WAS TAKING A HYDRAULIC HOSE OFF THE LOADER. HE WAS TRYING TO GET A FITTING LOOSE AND WAS BEARING ON IT. WHEN THE FITTING BROKE LOOSE, HE FELL AND HIT HIS HEAD ON THE LOADER.

EE STATES THAT DURING A LIGHTNING STORM THE POWER WENT OFF AT THE CONE CRUSHER. WHEN HE TRIED TO RESTART THE CRUSHER, THE POWER BOX SHORTED AND THE DOOR BLEW OPEN AS A FLASH OF FIRE CAME OUT AND SLIGHTLY BURNED HIS CHEEK AND NOSE.

EMPLOYEE WAS LOADING TRUCKS FROM STORAGE BIN WHEN A ROCK APPARENTLY FELL OFF A TRUCK AND HIT HIS FINGER.

EE WAS USING A GRINDER TO GRIND A SPINDLE ON A EUCLID AND A PIECE FLEW OFF THE GRINDER AND HIT HIM IN THE STOMACH.

EE STATES THAT HE WAS COMING OUT OF PIT ON A MODERATE INCLINE LOADED WITH ROCK ON HIS FIRST LOADOF THE DAY. THE EUCLID STALLED AND HE APPLIED THE EMERGENCY BRAKE. IT HELD ON FOR AWHILE THEN ST ARTED ROLLING BACKWARDS AND RUN UP ON A SIDE ENBANKMENT AND EASED OVER ON ITS PASSENGER SIDE.SEAT BELT WERE UNAVAILABLE BUT SOME UNEXPLAINED REASON THE EE WAS NOT USING IT.

EMPLOYEE WAS WORKING ON A WATER PUMP. THERE WAS WATER IN THE PIT. HE USED THE LOADER TO GET OUT TO THE PUMP. EVERYTHING WAS WET. HIS FOOT SLIPPED ON THE LOADER AND HE FELL AND FRACTURED HIS RIGHT ANKLE.

EMPLOYEE WAS TRYING TO GET TO A WATER PUMP TO WORK ON IT. HIS FOOT SLIPPED AND HE FELL, GRABBING ONTO THE HAND RAIL. HE HIT THE WALK ON HIS RIGHT LOWER SIDE NEAR KIDNEY AREA. RECEIVED A 3"" LONG CUT AND BRUISES.
