Employee was trying to repair lights on top of skid steer. Employee was standing on the entrance exit platform attempting to bend the cab shield up to repair lights. Wrench slipped causing employee to fall onto the bucket dislocating right shoulder.

EMPLOYEE CLIMBED UP ON LOADER TO REPLACE WIPER ASSEMBLY. EMPLOYEE HAD WINDSHIELD WIPER ASSEMBLY IN HIS HAND. EMPLOYEE STEPPED OVER ON FRONT FENDER AND FOOT SLIPPED OFF FENDER STEP AND EMPLOYEE FELL AND FRACTURED HIS RIGHT HEEL.

Employee was exiting the cab of the drill. He stepped down on the 1st step, then missed the 2nd step & fell about 4', landing on the ground. He stated that he was unable to grab the handle in time to keep him from falling. Other than failure to maintain 3-point contact, there appear to be no contributing factors causing the incident (moisture, wind, defective equipment, etc.).

EE WAS DUMPING A LOAD FROM HIS BACK DUMP WHEN THE LOAD SHIFTED BREAKING THE SAFETY CABLE AND ALLOWING THE BED TO OVEREXTEND CAUSING THE CAB TO RAISE OFF THE GROUND. THE TRUCK THEN BROKE IN TWO PARTS FORCING THE CAB TO FALL TO THE GROUND CAUSING EE TO HIT HIS HEAD ON THE WINDSHIELD.
