Picked up fifty pound bag of anfo to load anfo truck. Employee stated that he had an old hernia which was repaired three years ago. He stated that he re-injured the old hernia site while lifting the fifty pound bag of anfo. Surgery to repair new injury on March 16th 2004

UPON ARRIVAL AT INTERSECTION #2 OF AIR SHAFT ENTRY A ROOF FALL WAS OBSERBED THAT HAPPENED OFFSHIFT DURING NIGHT. FALL WAS OF #2 INTERSECTION APPROX 9' THICK. WATER WAS OBSERVED COMING OUT OF F ALL AT SAME LEVEL WHERE FALL STOPPED. WATER HAD BEEN BETWEEN LAYERS OF ROCK. TOP WAS BOLTED WITH5' BOLTS INTO SANDSTONE. SANDSTONE HAD SHALE STREAKS WITHIN ITSELF
