Mechanic was using a chain hoist, working on a Cat Artic truck, as they were working the handle on chain hoist they pulled a muscle in their left forearm
Miner was performing mechanic work under a service truck. EE over-exerted self in an awkward position.
A miner was lifting part of a cut up 55-gallon metal drum used as an oil catch from the ground to place it in the back of a service truck. After the shift ended, the miner notified EE's supervisor that EE had hurt EE's arm while lifting the oil catch earlier that day.

Miner lost footing in loose material while standing on top of finish screen deck during maintenance project. Miner was attempting to dismount screen via fixed ladder mounted permanently to the screen hood and lost footing as miner turned to descend. Miner was restricted for one day to no work the next day (Thursday.)

Employee was detaching trailer from truck. When the trailer hitch disconnected from truck ball the trailer shifted forward, pinning the employee's right foot to the truck's bumper.

Employee had been sanding motor shafts, turned to walk away and tripped over a saw horse and fell onto hands. Fractured Left Wrist.

Employee was re-spooling a cable winch. Employee was using left hand to guide the steel cable onto the spool. EE's left thumb got too close to the spool and was pinched by the cable and spool, causing a tear/laceration to the left thumb.
Helping install a hold-down roller on a conveyor. Employee was watching co-worker use a hammer and chisel to seat a bearing onto the roller shaft. As the co-worker struck the chisel with the hammer, the employee was struck in the right eye by a flying object (small piece of metal)

Our T371A CAT haul truck was hualing rock from underground mine to a stockpile located at the surface. While backing up to the stockpile the haultrucks brake chamber failed causing the truck to roll back a slight grade into the stockpile. The driver complianed of back pain, but finished out his shift.
Accident occurred while removing brush from beaver dam at quarry lake drain, slip and fall causing back pain the next day.
