Grabbed a chain to bind a piece of equipment down and felt something pull in his back. did not report accident until 9/28/05

A roof fall occurred 8 breaks inside in the belt entry. It measured approx. 30'long, 19'wide and 8'thick.

A roof fall occurred 1 break outby spad 194 and it measured approximately 80' long, 80' wide and 4 to 4.5' thick.

Roof fall 25 feet inby spad 71 - 4 ft thick 18 feet wide and 60 feet long.

Roof fall at spad 58 4 feet thick, 18 feet wide and 60 feet long.
