A 6x6"" timber fell striking employee's left index finger, fracturing the finger. Employee continued to work until 3-18-2015.
While installing a 5' torque tension bolt a piece of draw rock approximately 19.5"" long, 13"" wide, and 4"" thick fell onto employees' foot.

A roof fall occurred in the #3 entry, 20' inby spad #2720 and extending approximately 70' to the face and also into 3 right crosscut for a distance of approximately 20'.

While using a wire rope to pull a continuous miner conveyor chain together employee's hand was caught between the rope and the foot shaft of the miner, causing a laceration and fracture of his left ring finger.
Attempting to put gravel car back on track. Jacked up car and using a chain ratchet to pull car over when jack slipped out striking his left finger requiring glue.
Employee was attempting to open the bypass valve on a plaster sprayer when the pressurized material struck him in the eyes and face.

Employee was heating metal when a piece of rust popped loose and went under his safety glasses striking his left eye. He had the foreign object removed and was prescribed medicated ointment.
EE was using one scoop to push another scoop out of a dip when the bucket of the scoop fell off the other scoop resulting in his head hitting the canopy. He continued to work until 9-12-11. Accident was told by injured to not have happened on the job. A medical review was conducted and determined on 11-2-11 to be work related. Safety Dept was notified on 11-8-11 of the review.
EE was drilling a hole to install a roof bolt when a piece of draw rock 9""X9""x3/4"" struck him in the left side of his neck causing a laceration. He was taken to Pikevill Medical and received 11 stitches to close the wound.
Employee was checking a discharge line for a pump and discovered the fall in the #6 return entry that was approximately 70'L X 20""W X 7""H. All entrances to fall area have been breakered off.

EE was in the bath house cutting plastic from his lunch bucket and the knife slipped and cut his hand. He was taken to ER and received 6 stitches to close the wound. He was released to return to work.
At appx. 3:30 PM, an unplanned release of methane gas occurred in the No. 6 entry of panel 2A, inby survey station #1156, of the Northeast Seb-Mains of the above reference mine. A contributing factor to the release was a drop in barometric pressure.
A roof fall appx. 30'x19'x6'8"" was found on 5-19-2010 in the #6 entry at spad # 810 appx. 2600' from the active face. The roof in this area is mixed and is supported by fully grouted resin bolts and 8"" cable bolts. The fall did not affect ventilation nor was any equipment or miners affected by the fall.
Member had moved bolter up to begin bolting #3 right crosscut. While on down on one knee a piece of draw rock appx. 26""x14""x1"" fell from between bolts and struck member on head breaking in two pieces and one piece hit his left calf causing a laceration.

While cleaning scrubber in the #0 entry on #1 section a piece of drawrock fell out between rib and roofbolts. The drawrock struck member on head and neck. Member was attended to on the section and transported outside and taken to hospital by ambulance.
Injured EE & another EE were preparing to off load vulcanizing tools from a Johnson Industies 4 wheeler onto a rail mounted flat car. As the other EE was backing up to the flat car, injured EE stepped between the 4 wheeler and flat car, pinning his left leg between the two. Resulted in a fracture to his lower leg.
While operating a Joy 21SC shuttle car, EE ran car over a rock measuring 13""x6""x7.5"" causing him to jam his head into the canopy of the shuttle car. He reported he was having head and neck pain and was transported outside and then to hospital by ambulance.
While putting belt structure together, a rail fell and hit EE on right hand and wrist. He took himself to the doctor and received stitches. There were no witnesses to the accident and it was not reported to the operator until 08/20/2009.
While installing a cable bolt, a piece of draw rock fell striking the center of the bolt causing bolt to flip up and stike member in mouth, cutting his lip and chipping a tooth.
# 3 Heading loading straps onto bolter. Sharp edge of strap cut through glove to injure finger. Taken to ER for treatment, stitches used to close wound.
Racking belt on 1st North West Mains #5 entry. On top of belt pulling back onto pile. Stumbled, fell, struck right shoulder on scoop ram.

Employee on 001 section, #1 entry; using a 4 lb hammer to dislodge drill steel from chuck on drill pot. He missed the steel, struck left knee with hammer. Completed shift, went to E.R., referred to Orthopedic specalist. Attended appointment 6-17-08, Ortho took off work for a week with follow-up.
