Employee was using an extension bar to move lever on VSI damper. While pulling down the bar set free from being stuck which cause backside of left hand to contact cover of VSI. Contusion to the back of the left hand. X-rays were taken on 6/10/2021. Notified of a fracture on one metatarsal bone on 6/11/2021. A cast was placed on hand at local Emergency Room on 6/11/2021.

Miner was operating a forklift to move a section of screw conveyor when EE noticed it starting to shift on the forks. EE then stopped the lift, engaged the parking brake and exited the lift to get a better look. While readjusting the load EE's finger was pinched.

Employee was loading a pallet into the Bemis palletizer when he dropped the pallet and pinched his finger against a metal strip causing a laceration to the 5th finger on his left hand. EE received 7 stitches and returned to work with no restrictions.

Employee LOTO his machine saw a belt not turning reached in pulled on the belt, the belt started in the wrong direction and pinched his finger causing a laceration and torn ligament. Employee was taken to ER and received seven stitches and was allowed to return to work with no lost time.

Employee was walking under a platform and bumped his head on a steel beam. This beam had 68" of clearance and although the employee tried to duck as he went under, he did not get low enough.The impact caused an acute cortical strain to the neck.
Employee was struck by a forklift traveling in reverse (hit by counterweight). Employee has 5 fractures to the right foot/ankle and one fracture to the left foot.
Swinging a sludge hammer to free up material at the volumetric feeder and his hand was pinched between the hammer handle and a angle Iron that was put in place to temporally hold up the feeders when they were being but into place. Three sutures to small finger on right hand.

Stepped off step at office and felt a sharp pain in his left knee. Within minutes the pain was so great that he was taken to the doctors. He was released to go back to work on light duty and was given Percocets for pain. At this time he has to go to a orthopaedic doctor for a CAT scan to see if he has a torn ACL, appointment is scheduled for 11/5/09.

Trouble shooting a problem with the top sheeter on a bagging machine. Employee hit the reset button and did not know that other employee was behind gated area. Arm on machine hit employee in the back causing bruising. Two days lost time - back to work regular job.
Employee was working on quarry water pump. He reached behind the pump to access bolts and felt a pulling in his groin. He continued to work until his surgery Aug. 26, 2008 when this became a lost time accident.
While shoveling out a elevator pit employee felt a burning and pain to his left groin. He went to the Doctor and has a Hernia. He was operated on 7/7/08 and will be out of work for about a week.
Employee was climbing down a ladder and thought he was on the last step. He had four more steps to go and fell backward and broke his left wrist.

Employee was cutting electrical tape from electric motor wires and knife slipped cutting his two fingers on the left hand. One stitch to middle finger.
Employee was shoveling under conveyor and when he turned he felt sharp pain in back.
Employee was bending over in the portable crusher in the quarry making repairs and his hard hat fell off his head. When he reached for the hard hat his head hit a sharp piece of steel on the edge of the crusher requiring four stitches.
Employee was grinding on the crusher hopper when a piece of steel hit the side of the hopper and glanced under his safety glasses and went into his eye. At the time he though it was dirt in his eye. The next morning his eye was getting worse and went to the doctor.
Employee was loading pallets of material onto truck and pallet struck bed of truck and board flew into cab of forklift and struck left forehead.
Installing a pipe on the elevator to the dust collector. The pipe was tack welded in place. EE was pulling on the pipe with a comealong to line it up. When the weld broke and the pipe fell hitting his finger between the pipe and the elevator. 12 stitches.

Employee was mowing lawn and holding tree branches back with one hand and tractor tire hit diviot and pulled steering wheel from his other hand.
Employee was adjusting fork setpoint on palletizer setpoint. When locknut was loosened the fork slipped and pinched his right thumb.
Employee was enlarging pilot hole in a guard he was building with a electric drill when bit caught and forced drill from his hand. The drill spun around and the handle struck his right wrist. He broke his 4th Meta carsal bone in his right wrist.
Snow covered plastic tarp and was unable to see end and walked on snow covered tarp and slipped. Employee has had previous hip injury outside work.

When grinding a bushing apparently a piece flew up and cut Mike in the neck, just below his chin. Mike had a laceration 1 1/2" long which required 9 stitches.
Truck driver employed by Hildebrandt Tkg. Inc., Contractor ID #7DD, was strapping down his load on Oldcastle Stone Products property and was struck by a forklift operated by an Oldcastle employee in the right thigh.
CREW NEEDED TO CHANGE DUST COLLECTOR BAGS. DOOR HAD FROZEN MATERIAL AROUND EDGES. AFTER CHIPPING ICE AWAY WITH BAR EE TRIED TO OPEN DOOR. THERE WAS NOT ENOUGH MATERIAL REMOVED AND HE DECIDED T O DROP DOOR. DOOR DROPPED AND THE BAR EJECTED INTO HIS LEG AND THEN INTO HIS EYE.
JUMP STARTING EUCLID TRUCK ONE OF THE CABLE CLAMPS ACCIDENTLY SHORTED OUT AGAINST FRAME AND BATTERY EXPLODED.

DRIVER SLIPPED OFF FLATBED WHILE GETTING DOWN FROM TRAILER AFTER TARPING LOAD. LOAD WAS PARTIALLY LOADED AT ONE AREA AND HE MOVED TO SECOND AREA TO GET REMAINDER OF LOAD. WHILE WAITING FOR SEC OND PART OF LOAD, HE DECIDED TO TARP LOAD.
REPLACING BEARING ON QUICKLIME ELEVATOR, GOT QUICKLIME DUST ON CLOTHES. ON HIS WAY HOME THE DUST REACTED WITH SWEAT TO CAUSE IRRITATION AND BURNS ON HIS TORSO AND ARMS.
