Employee of Adams Trucking was weedwacking brush. Blade on weedwacker hit guide wire for electrical pole. The wire was severed and the tension pulled it back toward the transformers on the pole. the guide wire came in contact with a bare wire. At this time we assume electricity went to ground causing fatality. Under Investigation.

While walking into a building, a miner slipped on a walkway and attempted to brace themselves to prevent a fall. Miner was able to stop the fall but reported right shoulder discomfort. Physician approved to continue work without restrictions. After continued soreness an MRI resulted in a slight tear on 5/29/2026. Operator received documentation on 6/2/2026.
Employee pushed up safety glasses repeatedly, got some emulsion on their nose from gloves. EE developed small blisters later in the evening. Went to clinic two days later, diagnosed with 2nd degree burn.

Miner Injured moving a 55-gallon oil drum on dolly. Drum was not set properly on dolly and miner adjusted drum. While adjusting drum right pointer finger was pinched between drum and dolly resulting in a hairline fracture and laceration. Gloves were worn.

Injured EE reached for falling screwdriver. Index finger caught between screwdriver & equipment housing. Resulting in fractured index finger.
During the installation of a rotary valve to a hopper the injured EE lacerated left pointer finger from a pinch injury between the two flanges of the equipment being installed. The EE was reaching between the flanges to apply a gasket material when load swing occurred shearing the EE's finger between the two components.

Kiln building location. Right eye injury: employee was wearing goggles in the kiln building. The goggles had some hot lime residue on them. EE removed goggles, and a small amount of hot lime entered EE's eye causing a burn.
The injured EE suffered a hand laceration while cutting a piece of steel (ball screw) while cutting the screw it snapped and cut the injured EE right hand resulting in stitches to the EE's right hand index finger knuckle
The Injured EE was carrying a five gallon water bottle for the water cooler up a flight of stairs when EE strained a muscle in EE's lower back this incident was reported on 9-12-22, the EE did not seek medical treatment until 9-23-22. Incident recordable on 9-23-22
Employee was walking down stairs. Heel of boot got stuck on grate and employee tripped and fell down 6-7 stairs.
EE attempted to retract a magnet carriage into place. The carriage did not fully retract as prompted. EE performed LOTO and opened the hatch to the magnet to then find the magnet half closed. EE attempted to maneuver the carriage to return it to an open position simultaneously the cylinder actuated closing the carriage on EE's right hand. EE suffered a contusion.
Employee was moving a waste hopper via forklift to a dumpster. They dismounted the forklift and placed a hand at the base of the hopper and then released the hopper latch. The hopper rolled over left hand, lacerating middle finger. Employee was taken to the hospital where two stitches were applied.
EE was retrieving a piece of plastic hose that was below a catwalk on the roof of the building. EE used a broom to retrieve the hose. While pulling the hose over the guardrail of the catwalk towards self with right hand, frozen material shifted within the hose pulling EE's right wrist outward causing sharp pain. EE was diagnosed with a strained wrist.

While unplugging a mill circuit, the miner removed a guard, and the motor belt contacted miner's right thumb and severed it at the knuckle.

The employee snipped the bands off a pallet located in the bed of a pickup with a wire snips and felt a twinge in their back. At first appointment on 12/5, the provider restricted the employee to desk work until an MRI was completed, but did not indicate it was work related. The neurologist provided a report on 2/5 that said it is work related.
Right index finger tip pinched between v belt and a sheave during a belt change. The injury was a fracture which needed surgical repair so the employee lost three workdays while getting medical attention. The employee returned to work on 8/12/19.

The employee was operating the sand truck and pulled over to inspect the level of the sand hopper and employee's right small finger contacted a sharp piece of metal on the dump truck body. Employee's finger was lacerated and required sutures.
Employee bent over to pick up an empty plastic bottle, 4 oz., that had dropped, and felt a sharp pain in the back. EE reported to the supervisor and went to an urgent care facility that took them out of work. Then EE went to preferred provider back specialist who said EE could work with restrictions and that the injury was not work related.

Operator was instructed to clean out G-15 dust collector with the vacuum truck. EE was returning from evaluating the job and ee began descending down a steep set of stairs. The operator's left foot came down on a step 3-4 inches below what ee expected. The jarring impact fractured the employees heel.

The employee was changing an oil gun from a kiln and EE's right wrist was burned from hot kiln exhaust under the cuff of EE's glove.
Employee slipped and fell on oil residue. As EE was falling EE's right arm hit a metal guard resulting in a laceration. EE was taken to the local hospital where EE was treated and released without limitations.
While employee was attempting to open a fuller valve with a cheater pipe the pipe hit a beam. This caused kiln dust to fall from the beam into the left eye of the employee. The employee was treated by a local eye doctor and was given polysporin Ophthalmic Ointment. EE returned to work for EE's next scheduled shift.
Employee injured right pinky finger when a motor EE was positioning onto a shelf came in constact with a stationary motor on the shelf. EE's finger was pinched between the 2 motors causing a laceration. EE was taken to a local clinic and received sutures to the finger
Worker was scaling high wall using rope access methods. EE stacked excess rope (tail) on the ground above EE. EE needed more rope & pulled the tail and the tail dislodged a small rock. Rock hit EE in the nose. Ground area where rope was stacked was not cleaned properly.
This is being reported as a non-employee injury. A driver for New England Pallet Company loosened a buckle on a strap and threw the strap over stack of pallets on a flatbed truck. A gust of wind blew the buckle and strap back toward non-employee hitting non-EE in the nose and eyes. Non-EE has a broken nose in two spots and one of non-EE eyes has a damaged lens.
Employee felt pain in right shoulder after as EE was climbing up a stairway with a vacuum truck hose. On 02/10/2016 EE was evaluated at a local clinic and was released to work. EE continued to work and had an MRI that revealed a tear at which time the doctor put EE out of work on 03/28/2016. EE is currently awaiting a surgery date.

The contract employee was onsite to replace railroad ties. The employee's denim clothing was wet and plastered with mud which contained some lime. The employee developed a rash and was prescribed a pain killer, which he did not take. According to his employer, Railworks, his rash became infected and the Dr. recommended that he not return to work until it was fully healed.

While walking on snow covered ice from the Quarry Lunchroom the employee slipped and fell backwards injuring his left shoulder and right hip. No medical treatment initially. On 01/16/2014 he was evaluated at local medical facility and released for full duty with a referral to physical therapy (P.T.) Incident became recordable on 01/24/2014 when he began P.T.

Employee was loading refractory brick onto a freight elevator platform. He had placed his left foot under the platform. The platform was approx. 6 inches off the ground. As the employee was putting bricks on the platform the platform came down onto his left foot.
Employee was getting off of a fork lift and he caught the small finger on his left hand between the fork lift body and a door hinge pin. The momentum of his body going forward while his finger was caught resulted in a fractured finger. Employee has not missed work and is working with limited restrictions.
Two Mechanics were lifting a sheave onto a pick-up truck. As they lifted the sheave to the tailgate level one mechanic pinched his middle finger on right hand between the sheave and the tailgate. He received 4 stitches.

Employee was a opening a valve and felt a strain in his left shoulder. He continued to work without restrictions.On 11/28/12 he requested a medical appointment.During late January he reported that he is receiving physical therapy. He is still working full duty.

Alleged strain to left shoulder while climbing onto a forklift. Claim is contested based on physician's initial conclusion and second job held by employee. Further medical evaluation including a recent MRI reveals an old tear to the shoulder. Awaiting an opinion from an orthopedic surgeon. If proven to be work related the company contends this to be a repeated trauma illness.
Employee fractured left hand and bruised right hand while replacing gaskets on the secondary crusher. The bull gear hung up in the upper housing and came down unexpectedly as he was installing the retaining bolts pinching his hands between the lower shell housing and the retainer plate. Two by four blocks in place reduced the severity of the injury.

A non-employee entered Specialty Minerals' property and proceeded to the quarry area. The individual committed suicide by driving his 1998 GMC truck over the edge of the quarry. The vechicle fell approximately 300 feet to the floor of the quarry. The Massachusetts State Police investigated the incident and confirmed that the individual committed suicide.

Employee was packaging 50 lb. bags. He removed a bag from the spout and turned to the left to place the bag on a nearly full pallet. As he turned, he felt a sharp pain in his left leg and he fell to the floor. He received medical attention at the local hospital. He has a left knee strain and is working on restricted duty.

Employee was placing an empty propane cylinder into a cage used to transport the cylinders for refill. He pinched the 5th finger on his left hand between steel bracket in the cage and the propane cylinder. X-rays showed a fracture to the finger and he was placed on restricted duty.
Employee was found unconscious on a landing at the bottom of a set of stairs in the Screen Plant building. CPR was performed until EMS arrived. Employee was transported by ambulance to the hospital where he was later pronouced dead.
Employee was passing a cover for the pre-grind dust system to another employee and felt pain in his lower back radiating down the right side. He continued to work full-duty until 9/20/07 when he received medical attention and was placed on restricted work.
Employee was digging a stripping shot. The bucket hit a large boulder that was covered up smaller stones. This caused the loader to come to a complete stop and the employee's upper body went forward then backward causing a whiplash type injury to his neck. Seatbelt was worn. He continued to work full duty. He was then seen by a doctor on 1/22/07 and placed on restricted duty.

Employee was walking in the alley of the yard and a couple pieces of lime blew into his left eye. He was wearing the proper PPE of Safety Glasses with sideshields. He was treated at Advanced Eye Care where the Doctor removed the particles from his eye and gave him a prescription medication.
Employee was knelling down cutting belting. The knife he was using went through the belt into his right thigh.
Employee was moving a steel wheel on a jack stand cart. The wheel hit the bottom of a discharge pipe and fell onto the employees left hand. The injury required six stitches.
Employee banged his elbow on the steelwork of a dryer while working on the fan for the dryer. The elbow became infected. He was evaluated by a local Doctor and given antibiotics for the infections. He missed one scheduled day of work and worked one restricted day.
Employee was fanning empty bags prior to placing them into the packaging machine. He has pain in his left forearm and right shoulder. Employee states that this is a condition that occurred over time.
Employee worked in the Kiln Building most of the day. He felt an irritation in his left eye but did not recall an incident that caused it. He worked on 1/27/06 with an irritated eye and was seen by an eye doctor on 1/28/06. A small piece of metal was removed from his eye.

Employee was climbing down the ladder of QT-3 haul truck. His foot missed the top step of the ladder. His left foot then was caught under the step second from the top. He fell backwards and was hanging upside down from the ladder. A nearby employee was there to lift him up almost immediately. He injured his lower left leg and was placed on restricted work duty for one day.
Employee removed a dust colletor bag that was completely full of dust. The dust left in the plenum above came down and hit him in the face. A small amount of dust got behind his safety glasses and into his right eye. He flushed his eye and then was treated and released at the local hospital. He was give a prescription medicaton.

After filling the seal water vessel under tank 112 employee stepped backwards down an incline and strained his right knee. He was seen at a local clinic and was placed on restricted duty.
It was very windy morning. Employee was reporting to work and had a foreign particle blow into eye as he was walking from the parking lot to the quarry lunch room.
An employee was aligning an auger shaft to a drive shaft in a confined area. When he picked up the shaft he felt a pull on the right side of his lower back. He was seen at the local clinic and give a precription for 800 mg of ibuprofen. He was also given a lifting restriction which he is able to work with. He has a follow-up appointment at the clinic on 11/29/2004.
Employee was cleaning floor with a water hose and slipped and fell. His left knee came in contact with a cement floor causing a contusion. He was seen at a local medical clinic and placed on restricted duty.
Employee reported an illness of repetitive trauma to his right lower arm. There is no specific date as this has happened over time. He continued to work full time through 1/25/05 at which time he went out surgery to correct this problem and a non work related problem at the same time.
Employee was climbing a fiberglass step ladder to install a new water line and the rear support leg of the step ladder split. Employee fell onto existing piping and injured his ribs. He was lowered to the ground by the employee he was working with. He was seen at a local clinic and is on restricted work.
Employee was working on A filter press, when he stood up he hit his hard hat on part of the machine. The next day he felt pain in his neck and shoulder. He went to the Dr on May 13th and was placed on restricted duty.
EE WAS LIFTING A 50 LB. BAG TO STACK ON A PALLET. WHEN HE TURNED TO STEP TOWARD THE PALLET HE FELT A POP, IN HIS LEFT KNEE. HE WAS SEEN AT THE E.R. AND THEN BY AN ORTHOPEDIC DR. & WAS PLACED O N RESTRICTED DUTY.
EMPLOYEE WAS UNDER THE HEAT EXCHANGE FOR KILN #4 ATTEMPTING TO REMOVE THE BELLOWS FROM THE HEAT EXCHANGE TUBES USING AN AIR HAMMER. ONE 30' TUBE LOOSENED IN THE TUBE SEAT AND FELL STRIKING HIS LEFT THUMB WHICH WAS HOLDING THE AIR HAMMER. HE RECEIVED SEVERAL STITCHES AT THE LOCAL HOSPITAL AND WAS GIVEN MEDICATION.

WHILE CARRYING A PIECE OF PIPE, HIT FOOT ON PIPESTAND AND FELL. PIPE LANDED ON LEFT THUMB, INJURY REQUIRED STITCHES. NO LOST TIME OR RESTRICTED WORKDAYS. INJURY OCCURED AT SPECIALTY MINERALS M INE NAME ADAMS.
EE WAS OBSERVING REMOVAL OF ICE HANGING FROM A ROOF AND FELT DUST BLOW INTO HIS EYE. HE WAS UNABLE TO FLUSH THE PARTICLE OUT OF HIS EYE AND WAS REFERRED TO AN OPTOMOLOGIST. DR FLUSHED THE EYE OUT. HE WAS WEARING SAFETY GLASSES WITH SIDE SHIELDS.

EMPLOYEE WAS DRIVING AN ARTICULATED DUMP TRUCK WITH APPROXIMATELY 18 TONS OF MATERIAL. AS HE TOOK THE CORNER AT THE MAINTENANCE SHOP TO GO UP THE HILL THE LOAD SHIFTED TO THE RIGHT AND THE DUM P BODY TURNED OVER ON THE RIGHT SIDE. THE CAB WENT ONTO 2 WHEELS THEN CAME DOWN. EMPLOYEE RECEIVED STITCHES TO FOREHEAD.

IT IS SPECULATED THAT HE WAS WORK OFF OF A 10 FT EXTENSION LADDER AND FELL APPROX SEVEN FEET AND HI T THE RIGHT BACKSIDE A HIS HEAD. HE WAS TAKEN BYAMBULACNE TO BERKSHIRE MEDICAL CENTER AND WAS SEDATED FOR OBSERVATION. AT SCAN TAKEN WAS NORMAL.
EMPLOYEE WAS MOVING A LARGE GUARD AND IT SLIPPED OUT OF HIS HANDS AND FELL ON HIS RIGHT FOOT. HE WAS WEARING STEEL TOE BOOTS BUT HE INJURED THE TOP PART OF HIS FOOT. XRAYS WERE NEGATIVE. HE HA D ONE DAY OF RESTRICTED WORK AND THEN RETURNED TO FULL DUTY.
DRIVING DOWN EMPTY PAVED ROAD WET FROM WATER TRUCK SPRAY, TRUCK IN FRONT STOPPED AND SECOND TRUCK SLID INTO IT. APPROX. SPEED 5 TO 10 MPH.
WHILE DRILLING OUT THE DIESEL GUN PART FOR #3 KILN LIGHT-OFF, THE DRILLED LIME PARTICLES BLEW BACK AND MADE CONTACT WITH EE S RIGHT FOREARM. THE LIME CALCIUM OXIDE STRUCK TO THE ARM AND DISOLV ED IN THE PERSPIRATION CAUSING SKIN BURNS.

AS HE WAS DIRECTING THE LOADER OPERATOR WHERE TO DUMP THE THIRD OF 4 BUCKETS IN THE MIDDLE OF THETRUCK, THE BUCKET CAME IN CONTACT WITH THE SIDE RAIL AND ROCKED THE DUMP BODY. THE EE ROCKED BACK THEN JUMPED OFF. HE INJURED HIS SHOULDER.
EE WAS ATTEMPTING TO MOVE A FULL PALLET THAT HADCOME DOWN SIDEWAYS ON THE MOVING CONVEYOR. HE STOOD ON THE CONVEYOR WHILE ANOTHER EMPLOYEE WAS AT FLOOR LEVEL WITH A PRY BAR. THE EMPLOYEE ON TH E CONVEYOR FLAGGED THE PHOTO EYE AFTER FREEING UP THE PALLET AND THE CONVEYOR STARTED AUTOMATICALLY PULLING HIS FOOT INTO A 3/4" OPENING BETWEENTHE CONVEYOR AND THE ROLLERS.
EMPLOYEE HAD JUST FINISHED STRETCHWRAPPING A PALLET. HE CUT THE STRETCHWRAP AND TUCKED IT IN AT THE BOTTOM OF THE PALLET. HIS LEF THAND CAME IN CONTACT WITH BAR ON DIAMOND PLATE STEEL TURNTABL E AND HE RECEIVED A LACERATION TO HIS KNUCKLE ABOVE THE MIDDLE FINGER ON HIS LEFT HAND.

EE WAS DIGGING IN THE MUCK PILE WITH A FRONT ENDLOADER & A ROCK SLID DOWN FROM ABOVE & HIT THE BUCKET & JARRED THE EE'S NECK. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 9/7 AT WHICH TIME HE WAS G IVEN A PRESCRIPTION FOR STEROIDS WHICH MADE THISCASE RECORDABLE.
EE WAS STEPPING OUT OF A DUMP TRUCK. AS HE WAS EXITING THE DUMP TRUCK HE STEPPED INTO A HOLE AND SPRAINED HIS ANKLE X RAYS WERE NEGATIVE PRESCRIPTION WAS GIVEN.
EMPLOYEE WAS CLEANING THE HOPPERS OF THE SUCKER TRUCK. THE WIND BLEW SOME HOT LIME DUST INTO HIS EYES CAUSING BURNS. HE WAS TREATED BY AN EYE DOCTOR AND GIVEN A PRESCRIPTION.
