EE was splitting stone, and the teeth were too high on the bucket of the excavator. The stone fractured and folded landing on EE's ankle, between two stones. EE was using a Chicago pneumatic jack hammer model CP4123

We sent employees for annual hearing tests and employee had a standard threshold shift in his left ear. He is working in a lower noise area.

Employee was splitting a piece of stone and the stone had a fracture in it, when the stone broke a piece landed on his foot.
