EE was splitting stone, and the teeth were too high on the bucket of the excavator. The stone fractured and folded landing on EE's ankle, between two stones. EE was using a Chicago pneumatic jack hammer model CP4123
Stone fractured when splitting
We sent employees for annual hearing tests and employee had a standard threshold shift in his left ear. He is working in a lower noise area.
Employee was splitting a piece of stone and the stone had a fracture in it, when the stone broke a piece landed on his foot.
Employee was replacing planks on our scale deck and strained his back.
Employee pinched his right middle finger in truck tail gate, half of finger is amputated. He returned to work 9/21/2010.
Employee was walking down a small grade from one quarry level to another level at the Beech tree pit. He had a burning sensation in his right knee and could no longer walk on it.
