Ashfield Stone Mfg, LLC · Over-exertion in lifting objects

Miner says EE was just cleaning up around the guillotine slab and when EE picked up a rock EE felt a "twinge" in EE's lower back. By the time EE drove home EE was in pain and went to hospital. EE didn't report to anyone until the next morning. EE had x-rays and was given a prescription for ibuprofen and a 7 day excused from work note.

ASHFIELD STONE LLC · Caught in, under or between a moving and a stationary object

Miner was about/preparing to clamp a 27" X 72" X 1 1/4" thick slab. Slab was not braced properly. Miner pulled slab against himself, pinched his thumb, moved back away and slab fell toward him. He fell as slab slid down his leg, eventually landing on his boot. His lower leg was fractured.
