Magruder Limestone-Troy Quarry: 175 MSHA citations Metal/Non-Metal

Employee was involved in NWEM Training and being tasked training an operation of a haul truck. EE was riding along with another employee. They stopped and pited the truck. When employee was climbing down from the the haul truck EE reports that EE missed the last step and rolled EE's ankle.

Employee is reporting bi-lateral carpal tunnel to wrist/hands from operating a Cat 992D pit loader. Employee is also complaining about shoulders hurting.

Climbing down from conveyor lost footing and fell about 2 feet onto catwalk landing on metal object.

EE reported EE was tightening a hydraulic hose with a wrench. While pushing on the wrench something popped in EE's elbow.

As the employee was climbing out of a bin the employee's foot slipped and employee's arm got caught on an old bolt causing a puncture wound and trauma to the arm.

Employee reported they were swinging a large sledge hammer while installing the gabion plate back in the grizzly. In order to do that they had to swing a large (heavy) sledge hammer to beat rocks out. *Employee did not report injury until 11/29/2021*

While installing pin for a hydraulic cylinder on a track hoe, the pin fell onto injured employee striking EE in the back.

Operating haul truck at asphalt plant hit upper arm/shoulder against door.

While working on a rock drill, employee used the upper arm, backside to push hoses up and away to move a bracket. Shoulder popped.

Employee was carrying ANFO bags from the truck to the shot, about 30 feet away from each other. As EE walked up a slight incline to the shot, EE felt a very painful pop in right calf muscle and EE immediately went to the ground. After sitting on the ground for a few minutes, employee got up to see if EE could walk and couldn't bare much pressure on the right leg without pain

Injured employee reports that they have bi-lateral carpal tunnel syndrome caused by 36 years of working at Magruder Limestone performing general labor, equipment operation, and various job activities. IE reports these include turning wrenches, hammering, changing screens, operating equipment, welding, shoveling, picking, jack hammer.

Raising hood on road dump truck and strained groin area.

Removing tire from rim strained right bicep.

Shop employee had heated a loader bucket tooth to remove, struck tooth with hammer resulting in a hot piece of steel entering finger.

While working from a ladder the ee was using a hammer to open the door at the bottom of a bin. As ee swung the hammer it caught an electric power cord which threw him off balance and he fell off the ladder.

Installing new training idler on the conveyor, the idler slipped down the frame of the conveyor a few inches and struck employee in the elbow.

Employee was climbing/stepping down from his position on a Volvo haul truck, employee's foot slipped, his leg twisted around as he caught himself and he strained/sprained a muscle in his groin area.

The employee was carrying bags of anfo loading a shot, the employee stepped on uneven ground, lost his balance and twisted his back.

Employee was loading a stockpile truck, employee stepped on a fixed mounting bolt, this caused the employee to be off balance and step down onto an uneven surface, employee rolled and twisted his ankle.

Employee was tightening a nut with an impact wrench using a wobble socket. The socket failed and a piece of it cut the employees thumb and palm on his left hand.

Employee was restacking deck boards on low boy trailer and ruptured his bicep tendon.

While climbing the truck ladder to truck cab the employee missed the top rung and fell to the ground. He landed on the ground on his feet but fell backwards and hit his head on the concrete pad he was parked on. He cut his scalp requiring 4 stitches to close wound. He returned to work the same day.

Engine hood of 900 Komatsu loader fell on employees head.

EE was holding chisel while coworker was hitting it with a sledge hammer. Coworker missed and hit EE's finger, resulting in a chipped bone at the end of EE's finger. This accident was reported to me on 03-18-05.

employee was preparing to get off of conveyor after splicing belt and fell about 15 feet to the ground. Employee landed on the side of his leg bruising his knee.

Employee was walking around his haul truck and walked into the rear tailgate, hitting his head.

EE WAS DRIVING THE 966 LOADER DOWN THE HAUL ROADAND IT RAN OUT OF FUEL. HE THEN LOST CONTROL DUE TO THE HYDRAULIC STEERING NOT WORKING. THE LOADER RAN INTO THE ROCK IN THE WASTE PILE AND TURNE D OVER ON ITS SIDE. EE WAS WEARING SEAT BELT AND SUSTAINED A FEW CUTS TO HIS SCALP AND BRUISES. HE RECEIVED 31 STITCHES AND SENT HOME.

MASHED FINGER BETWEEN TIRE AND SPINDLES WHILE CHANGING TIRE ON DUMP TRUCK.

EMPLOYEE WALKED INTO STEEL PART OF BREAKER, HIT HIS CHEEK JUST UNDER LEFT EYE, BROKE HIS CHEEK BONE, DOCTOR DECIDED TO PUT A STEEL PLATE ON BONE TO INSURE HEALING, WAS OUT 1 DAY FOR OUTPATIENT SURGERY AND PART OF 2 OTHER DAYS FOR DOCTOR APPOINTMENTS.

BUMPED HEAD IN TRK & THAT AGGRAVATED BACK INJURY

DRIVING HAUL UP TRUCK AND THE TRUCK RIDE HURT HIS BACK.

ROLLING UP USED 36" CONVEYOR BELT AND HURT BACK.

EMPLOYEE WAS LIFTING A HURT HIS BACK.

A ROCK WAS STUCK IN THE FEEDER AND EE WAS PRYING ON IT WITH A PRYBAR TO DISLODGE IT. THE BAR SLIPPED AND HIT EE ON THE SIDE OF HIS HEAD JUST BELOW THE EAR AND BEHIND EAR.

POPPED HIS BACK WHILE LIFTING HOOD (40 LBS) OF TRUCK. TOOK TO DOCOR AT HOSPITAL FOR X-RAYS. DOCTOR TOLD HIM TO REST 48 HOURS OVER THE WEEKEND.

WAS LOADING ROCK ON STOCKPILE TRUCK AND DUST GOT IN HIS EYE.

MISSED STEP ON TRUCK AND FELL TO GROUND.

WHILE LOADING A SHOT,EMPLOYEE WAS CARRYING A BAG OF NITRATE TO A HOLE,WHEN HE STEPPED ON AN ICY PATCH AND TWISTED HIS BACK.

WAS LOADING A SCREEN DECK ON LOW BOY. EMPLOYEE WAS USING A PRYBAR TO STEADY LOAD. PRY BAR SLIPPED AND SHOVED HAND INTO ANGLE IRON FRAME.

EE & OTHERS WERE CLEANING ROCKS FROM FEEDER. OTHER EE DROPPED A 40# ROCK AND IT MASHED EE'S FINGER.

WAS UNLOADING PARTS FROM TRUCK & SMASHED THUMB ON RIGHT HAND UNDER A PIECE OF METAL. SENT TO HOSPITAL - X-RAYS WERE NEGATIVE - THUMB IS LACERATED & DOCTOR PUT 3 STITCHES IN THUMB.

WRENCH SLIPPED OFF BOLT AS HE WAS REPAIRING THE DRIVE SHAFT ON A 988 A CAT HAUL TRUCK HE MASHED HIS RIGHT RING FINGER. TOOK TO HOSPITAL FOR X RAYS NOT BROKEN.

EMPLOYEE SLIPPED ON SIDE OF TRUCK & FELL INTO TRUCK BED

HE WAS WORKING ON THE END OF THE CONVEYOR WHICH WAS EXTENDED OVER THE RIP RAP PILE AND WAS APPROX. 20 FT. ABOVE THE PILE. THE BOLTS HOLDING THE SECTIONS OF THE CONVEYOR PULLED OUT AND THE END SECTION OF THE CONVEYOR FELL. HE FELL ONTO THE RIP RAP PILE.

WE WERE REPLACING HAMMERS IN OUR ROTOR. WE HAD REPLACED 1ST HAMMER & WERE TURNING ROTOR TO REPLACE NEXT ONE, WHEN EMPL'S FOOT BECAME CAUGHT.

CHAIN THAT CARRIES HAMMER BROKE AND OPERATOR WAS REPAIRING SAME. THE CHAIN SLIPED OFF OF SPROCKET AND MASHED THE END OF RIGHT INDEX FINGER.

THE RACK BROKE ON DRILL STEEL AND STEEL CAME DOWN AND BROKE HIS RIGHT WRIST
