Ashley Quarry Metal/Non-Metal

The mechanic employee was walking/carrying a track hoe seat from the equipment he was working on. He did not see a box on the floor and tripped on the box. He then fell against the track hoe, caught his finger nail and the nail came off.

Employee was attaching a band type hose clamp to a water pump. He cut the back of his left hand on the end of the clamp. The cut was between his middle and ring fingers. The location of the cut required stitches to keep the cut closed. Cut was stitched and employee returned to work.

EE WAS LOADING STOCK PILE TRUCK - HE LOOKED UP TO CHECK LOADING & A CHIP OF ROCK WENT INTO HIS EYE. WE HAD TO TAKE HIM TO DOCTOR TO REMOVE CHIP-NO DAMAGE TO EYE. HE WAS WEARING SAFETY GLASSES

EE WAS DRILING ROCK FOR BLAST HOLES WHEN A SLIVAER OF STEEL BROKE OFF THE THREADS OF DRILL STEEL& WHEN ABOUT 1" ENTER HIS UPPER RIGHT ARM. SICE IT WAS THAT DEEP & CLOSE TO MUSCLE THE DOCTOR DE CIDED TO OPERATE & TAKE IT OUT.

EMPLOYEE PINCHED MIDDLE FINGER OF LEFT HAND BETWEEN WRENCH & SIDE OF IMPACTOR WHILE HE WAS REMOVING KEEPER. SENT TO EMERGENCY ROOM FOR X-RAYS - NO BROKEN BONES - FINGER DID SWELL UP.
