Fall onto or against objects
Left forearm puncture wound, about 2cm deep, after loosing balance and hitting/piercing a 2"" piece of medal. Sought ER treatment, tetanus shot, x-ray, a couple sutures. Returned to work full duty after treatment.

CONDITIONS WERE DRY, OPERATOR WAS EXITING LOADER IN ORDER TO TAKE LUNCH, DID NOT GRAB DOWN HANDLE, MISSED 1ST STEP AND FELL APPROX 5 1/2 - 6 FT TO GROUND.
