Fall from machine

EE was exiting the loader to use the portable toilet on site when EE's foot got caught on the bottom step causing EE to trip and fall, landing on left shoulder resulting in multiple broken bones.

Struck against a moving object

Employee was attempting to make a road around a stock pile and when scraping with the 240 Volvo Loader he hit a rock causing the loader to stop quickly and the employee to injure his back. Employee has suffered a sprain to his back

Fall to the walkway or working surface

EE was standing on the ground below the finish deck of the crusher; pulling on a rope attached to an eyelet of a 40x60 sect of tarp and in the process the eylet broke off the tarp & ee lost his balance & fell backward onto his back.

Employee was loosening a bolt using an air gun and applied pressure with the injured hand (left) with his palm but the bolt was not fully inserted into the segment, consequently when air gun pressure is applied to the nut it spun the bolt and injured the left hand (palm).

Absorption of radiations, caustics, toxic and noxious substances

Worker was attempting to jump start a portable crusher using jumper cables and a Volvo 150 loader. First he hooked up the cables to the crusher battery and secondly to a running loader and within a second the crusher battery exploded and sulfuric acid splashed on his face, glasses and clothing. He flushed his eyes on site and was taken to a clinic for precautionary measures.

Ee was putting anti-freeze into Linkbelt 4300 while the unit was running and the air pressure from the fan blew the liquid toward his face and into his eyes.

The employee bent over to pick up an old pair of ear plugs off the ground by the supply trailer and felt a sharp pain to his lower back. He sat in the truck for 1/2 hour and then reported it. Employee taken to Occupational Health & Rehab in Londonderry, NH.

EE WAS WORKING ON A JAW CRUSHER ALONG WITH OTHER WORKERS. THE TASK WAS TO REMOVE TIRES TO GET TO THE FROZEN BRAKES. WHILE EE WAS PICKING UP THE TIRE NUTS FROM THE GROUND, THE TIRE WAS DISLODGE D FROM THE HUB AND HIT HIM ON THE HEAD THEN THE LEFT SHOULDER. THE WITNESS WAS WORKING ON REMOVING NUTS ON THE NEXT TIRE USING AN HYDRAULIC TOOL & THE VIBRATION OF THE TOOL MAY HAVE CAUSED TH
