Warren Sand & Gravel Inc · Fall from machine

Folding discharge conveyor section 1 miner was going to attach chains to head pulley for lowering. Bolts were held in place by tension only when movement was applied bolts loosened/fell conveyor collapsed. Miner fell 7' +/- to ground landing on lower back/hip and hitting head also, brief unconsciousness.
