EE was exiting a screen deck when EE stood on the door that had become wet from light rain which made it slippery and EE's feet slipped causing EE to fall straight on butt causing severe back pain. The EE had called another EE to come help finish the job but EE had fallen before the second EE got there. Has a t12 and rib fracture.

Emp was using a digging bar to help loosen a tire on a haul truck. Bar slipped and struck employee in the shoulder area.
Insured worker and another emp were unloading 40ft long pvc pipe when emp lost handle on pipe and it hit him on his right knee. Emp missed 3 days of work. Emp returned to work on 10/13/12.
Emp was installing wear rubber on vsi when lid was lowered, pinching his right forearm.

Emp. reported he was working on equipment and felt a pain in his arm, like a bug bite. emp did not go to doctor until 7/31 and was given a note with no restrictions or medical treatment. on 8/12/2008, emp still felt pain and returned to the doctor, and on 8/13 emp had a non metal object removed from his arm. medical treatment and four stiches.
EMP WAS USING A UTILITY KNIFE, CUTTING TOWARDS HIMSELF AND CUT HIS KNEE. EMP MISSED 1 DAY OF WORK AND HAD ONE SUTURE.
EMP WAS CHANGING SCREENS AND BUMPED HIS RIGHT KNEE. WHICH BECAME SORE. EMP MISSED 2 DAYS OF WORK, 5 DAYS OF RESTRICTED DUTY AND RETURNED TO REGULAR DUTY ON 4/28/08.
EMP FELL OFF A STEP LADDER WHILE PERFORMING MAINTENACE/REPAIR ON A STEAM JENNY. STEP LADDER WAS NOT FULLY OPENED, IT WAS PROPPED AGAINST EQUIPMENT.

Employee was getting cardboard blast hole casing from a shelf. He was turning to toss the casings to the floor and lost his balance. He fell approximately 3 feet to the floor.

emp stated his wrist was sore from beating ice off of steel. emp did not seek medical treatment until 12/26/07 where he was given a presciption and limited use of his wrist.
Emp was walking from truck to excavator, slipped on snow/ice and jammed right arm. Emp was given percocet and had to wear a sling for one work day.
Burn on left leg due to cleaning boots with steam jenny, EMP DIDN'T REPORT THIS UNTILL 10/26/2007.
EE was shoveling tunnel belt and felt pain in his back. EE took off work fri 8/31/07 without doctor note. EE was told he needed to see a panel doctor, who then put him on restrictions.
EMP WAS LIFTING 40-50 LBS ANVIL AND FELT PAIN IN HIS BACK. EMP MISSED WORK FROM 6/13 TO 6/15. EMP RETURNED TO WORK WITHOUT A RELEASE ON 6/18 AND WAS SENT HOME TO GET ONE. EMP RETURNED TO WORK ON 6/20 WITH RESTRICTIONS.

Employee was putting away a chain wrench and got his finger pinched between the chain and wrench.
Employee was dragging a 2" air line and stepped into a gulley and fell. He twisted his knee while falling to the ground and his head struck the ground. His hard hat flew off and his head hit the ground again, without his hard hat.
EMPLOYEE WAS OPERATING EUCLID 0029 WHEN DRIVE SHAFT CAME APART. WHEN THE DRIVE SHAFT CONTINUED TO SPIN, IT KNOCKED OUT BOTH REAR BREAKING CYLINDERS. THE TRUCK DESCENDED A GRADE, AND PARTIALLY UP ANOTHER GRADE, THEN ROLLED BACK INTO THE HIGHWALL.
