Coworker was walking up stairs to the parts room above the office when EE felt a pop in EE's knee. No slip, trip, or fall occurred. Coworker was evaluated at Nason Occupational Health and was prescribed a hinged knee brace.

A coworker was fabricating a feed box for a wash screw. The work was being done on EE's welding bench. An 18" by 20" plate EE had set on the top shifted and fell about 16", crushing EE's right thumb. The injury resulted in medical treatment.

EE had completed housekeeping under the 600 Crusher Feed Tunnel Belt and was exiting from the back side of the belt. As EE planted left foot, it slipped on the wet surface and EE fell to the ground, injuring lower back. The injury resulted in restricted duty.
EE was dismounting a utility truck bed with the tailgate in the up position. As they stepped onto the step bumper, left foot slipped off the bumper causing them to fall to the ground landing on left side. Following self-care measures, EE was evaluated by a primary care provider on 1/24/23. The left shoulder injury resulted in restricted work activity.

EE was painting the underneath steel structure of a lowboy trailer from a rolling creeper. When the EE stood up, they felt discomfort in right knee. The EE applied self care measures until they were evaluated by a panel provider on April 14, 2022 when the injury became reportable.

EE was repairing a rear strobe/tail light on a Tri-Axle truck. EE removed cut resistant glove from Left hand to separate light fixture from wiring harness. EE then attached new light fixture to the wiring harness and pushed light fixture into position when EE's left 5th finger struck the tailgate slider causing a laceration.
Winch got bird nest from cable being frozen on boom overnight and was in process of rewinding cable back on drum when crowbar slipped and EE fell back 4.5 feet of counterweights onto ground on outrigger pad mat.

EE was changing seat in a cat 777D rock truck. EE removed bolts from the seat, and lifted it to the ground using a crane. EE then took seat into shop with dolly cart. EE went to flip the seat over in the shop to remove the suspension from the seat body and felt discomfort in lower back.

A track hoe slack adjuster coil spring was being cut for scrap, during the process, a second employee (victim) was walking in the vicinity of the work. The center pin (inner portion of the adjuster) broke during the heating of the coil spring (outer portion) of the adjuster and struck the victim in the lower left leg while walking past, fracturing leg above the ankle.

EMPLOYEE WAS WALKING INTO BUILDING AND PUT HALF HIS FOOT ONTO A STEP AND TWISTED HIS RIGHT ANKLE CAUSING INJURY.
Employee was pulling motor from a dozer the majority of the day. It was work that caused him to kneel on the ground repetitively causing inflammation on his knee.
UNLOADING/LOADING TRUCK, UNLOADED A SHAFT AND SAT IT DOWN, MOVED CRANE OVER TO PICK UP ANOTHER SHAFT THEN WENT TO LOAD ON TRUCK AND TRIPPED OVER THE FIRST SHAFT AND FELL ON RIGHT SHOULDER
USING CHIPPER HAMMER ON MANGANESE, STEEL CAME OFF, WENT THROUGH LEATHER GLOVE AND HIT LEFT THUMB CAUSING PUNCTURE/INJURY.
Employee had a tinted visor on to burn hole, he took off the visor to install a bolt. He stood up and hit his forehead on metal stand causing a laceration.

Employee was running a chain saw when he lost his footing. The chain saw cut his knee, he had to go to the hospital to get stitches. He was released for full duty afterwards.
EE was using a pry bar that slipped and hit his mouth, breaking his tooth. He is getting it repaired by a denist.
