The coworker was carrying 2 flat plates of steel weighing approx. 22lbs combined weight when they felt a pain in right shoulder. They stated that it felt like they were stung with a rubber band. Their shoulder began to bother them and their fingers were getting numb. Health line was contacted and was advised to seek medical attention.

emp was making repairs on plant and was holding onto an i beam, emp felt his right wrist pop. emp reported the claim on 9/26 as record only but on 10/1 made an appt to see dr on 10/02. emp has a broken wrist
Emp was removing snow and ice from scale area when he slipped on ice and fell. Emp's right hand hit a concrete barrier on the way to the ground and he received a laceration needing 2 sutures. Emp returned to work on 1/13 with limited use of right hand. Emp followed up with doctor on 1/14 and released to work with no restrictions, normal duty.
Emp. was pulling a cable and had his knee in place for support, when he began pulling, he felt a pain in his knee. Emp did not seek medical treat at the time of the accident, but later went to er for swelling and soreness. Emp missed 1 day of work, but is still on light duty.

Emp stated he got out of bin truck and felt a pop in his leg. He did not report accident at that time, but did not show up for work on 1/29/07. He did not report injury to me until 2/2/2007. EMP ALSO STATED DID NOT FEEL PAIN UNTIL SUNDAY OR MONDAY.

EMPLOYEE WAS WORKING ON CONVEYORS AT CRUSHER. HE WAS PUSHING ON A 4 FT LENGTH OF PIPE AND A RATCHET ATTEMPTING TO LOOSEN BOLTS. AS HE PUSHED HE FELT A BURNING SENSATION IN HIS GROIN AREA.
WORKER STARTED MANLIFT. HE ATTEMPTED TO MOVE THE BUCKET. BUCKET WAS A GROUND STUCK IN ICE. BASKET SUDDENLY BROKE FREE FROM ICE. BOOM SPRUNG FORWARD/UPWARD LIFTING WORKER APPROX. 5 FEET INTO TH E AIR. WHEN HE CAME DOWN, HE HIT THE RAILING ANDLEVERS OF THE LIFT.
DRIVER WAS REPAIRING TARP SYSTEM ON TRUCK, CUTTING A PIECE OF METAL TUBING WITH A HACKSAW AND VISE. AS HE FINISHED CUTTING THROUGH THE TUBING HIS RIGHT INDEX FINGER HIT THE CUT EDGE OF THE SAW . HE RECEIVED STITCHES.
