Bundy Auger Mining, Inc. · Caught in, under or between a moving and a stationary object

While performing EE's duties as the auger groundman, EE's index finger was accidentally caught between 2 drill steel sections. While swinging the 5th steel into place it slipped down catching EE's right index finger.

Black Hawk Mining Inc · Caught in, under or between a moving and a stationary object

Placing a wood block under the auger machine. Got his left hand pinched between the block and the auger machine. Smashed two fingers, left hand middle and ring fingers.
