Employee was working under wash screens to pull the drain plug out of the concrete screw. The drain plug was frozen and the employee walked back to the maintenance truck to get tools. EE slipped on the ice and struck their head on the ground below.
Employee was shoveling down in the surge tunnel and struck finger against a tail pulley guard.

Welding in S-3 discharge chute. Lowered piece of 16" x 2' patch down to the tacked into place. Was then welding in chute between EE's legs and sparks caught pant leg on fire.

Employee was cleaning around the plant in a skid steer working around the surge pile tunnel at the quarry, the weather was wet and the employee was dumping a bucket load of wet sand on to a spoil pile near the tunnel. As the employee dumped the load of material it splashed back and the material splashed on the employee's face running down behind their safety glasses.

There was a buildup of frozen sand on the rail car door that two employees had to remove in order to properly close the door. After the material was removed, an employee inserted a spud bar into place on the car door and began to pull the bar towards self with a staggered stance. While pulling the spud bar back, the employee felt sharp pain around ankle.

Employee was preparing to line up a bolt on the wash screen. The bonnet of the wash screen moved forward which resulted in the employee "jerking" their hand away, which resulted in the employee suffering a laceration to their right index finger. The employee received 7 to 8 sutures to close the wound.

An employee working for a contractor was using a 6" inch DeWalt angle grinder on the corner of a new cold fee bin to prepare to weld. When he began to grind the surface in a vertical fashion, the grinder "kicked" back at the employee striking him above his right eye and on the right side of his nose.
"Pulled up with Terex (26.0012) to fuel. Employee removed the fuel cap. Stuck the fuel nozzle in and went to turn pump on. Went back to location and squeezed the trigger and the nozzle popped out and sprayed ee on the top of the hard hat and it entered into his right eye".

Operator had his bed raised washing material out of got back in truck and failed to lower bed before pulling away and caught the power lines and snapped electric pole. We left operator in machine until power could be terminated.
Had a fire start in mill control room that lasted a short time. We installed a new control room. Fed the power from a different transformer bank, and ran new power supply.

Employee & co-worker were patching a railcar from underneath the car, it was a tight spot, when 2 small pieces of rust got behind his safety glasses & became lodged in his eye. We were unsuccessful in removing the specks with water so he was taken to an eye dr. who removed it. He was prescribed eye drops as a precaution for any eye infection. He then returned to work that day.
Employee was clearing a rock that had become wedged between the tailgate & the bed of an R-65 Euclid haul truck using a long pipe. When the rock came loose it fell onto the tire below & bounced towards EE hitting him in the face. He was taken to the ER where he received 10 stitches to his upper lip.
EE drove his truck down to the lower bench and stopped his truck 100' behind and to the left of thr 992G Cat loader, in a blind spot wher the loader operator (injured) could not see him. Not knowing truck was in area injured cont. to back drag floor for 2nd operation. Truck driver seen loader backing up darted truck forward in path of loader hitting tailgate of truck bed.
While working on front-end loader with a wrench in his left hand he reached down to loosen a hose nut, when it broke loose his hand moved across a section of the hose that had a double clamps with one end sticking up and one down. The top of his hand racked across a sharp end of a clamp end cutting it to the point to require stitches.
While backing an R65 Euclid truck up a ramp to dump fill material, the left rear tires went into a low spot causing the truck to dip. Employee said this caused him to be thrown against the door handle of the truck. We do not know if he was wearing a seat belt. This injury did not become reportable until he had surgery on Sept. 29, 2006.

The employee was opening railcar doors to clean the cars. One of the door latches was stuck so he pried up on it with a spud bar and when he did he felt a sharp pain in his back left shoulder. He did not seek treatment for this injury until 8-1-05. He was given a prescription for physical therapy for 3 times per week for 3 weeks.
EE WAS WORKING HIS REGULAR JOB ON THE RAILCREW, WHEN SOMETIME DURING HIS SHIFT DUST BLEW INTO HIS EYES. THE WINDS THAT DAY WERE BLOWING EXTREMELY HEAVY AND ALTHOUGH HE WAS WEARING SAFETY GLASSES THE CONDITIONS CREATED A SITUATION MORE THAN THE GLASSES COULD OBSTRUCT. HE RECEIVED EYE DROPS AND AN ANTIBIOTIC FOR AN EYE INFECTION THAT LATER DEVELOPED.

EE WAS WORKING ON THE RAIL CREW CLOSING THE AIR BREAK SYSTEM WHEN HIS HAND SLIPPED WHILE TURNING A VALVE. WHEN HIS HAND SLIPPED HIS BODY PUSHED FORWARD ONTO THE KNUCKLE OF THE CAR INJURING HIS RIBS. ALTHOUGH THE INJURY OCCURRED ON THE 19TH, HE CONTINUED TO WORK THE REST OF THE WEEK. HE DIDN'T MISS WORK UNTIL THE FOLLOWING WEEK AND WDI WASN'T NOTIFIED OF THE SITUATION UNTIL 8/26/03.
EE WAS CLEANING UP STONE IN THE "OLD TUNNEL" USING A SHOVEL. WHILE SHOVELING STONE FROM THE FLOOR AND PLACING IT BACK ONTO A CONVEYOR HE FELT SOMETHING PULL IN HIS STOMACH. HE HAS RETURNED TO WORK WITHOUT MISSING ANY TIME BUT WITH RESTRICTIONS. HE WILL REQUIRE AN OPERATION TO FIX THE HERNIA IN THE FUTURE.
EE FIRST INJURED HIS LEG WHILE CHANGING ROLLERS ON A STACKING CONVEYOR WHEN HE WALKED INTO THE NEW ROLLER THAT WAS BEING MADE READY TO BE REPLACED. HE COMPOUNDED THE INJURY LATER AT 10:00 P.M. WHEN HE BUMPED THE SAME LEG ON THE C500 CRUSHER WHILE PUTTING ON SIDING.
EE WAS CUTTING A GUARD SUPPORT WITH CUTTING TORCH WHEN HE GRABBED THE HOT ANGLE SUPPORT THAT HE HAD JUST CUT WITH HIS HAND. HE WAS NOT WEARING GLOVES AT THE TIME OF THE INJURY.
SOMETIME DURING HIS SHIFT HE GOT A FLAKE OF METAL IN HIS EYE. HE WORKS OUTSIDE ON THE RAILCREW & IT WAS A VERY WINDY DAY. HE IS NOT SURE WHEN THE FLAKE BECAME LODGED. IT HAD TO HE REMOVED BY A DOCTOR.
EE WAS WORKING IN THE #2 BIN, KNEELING ON THE 304 CHUTE, LIFTING GRIPSTRUT TO PLACE UP FOR A WALKWAY. HIS SUPERVISOR WAS NOT NOTIFIED OF THE INJURY UNTIL THE NEXT DAY, JANUARY 30, HE DID NOT GO FOR MEDICAL ATTENTION UNTIL MONDAY, JANUARY 3

EE WAS WORKING IN OUR 'OLD TUNNEL' PICKING UP ROCKS THAT HAD FALLEN OFF THE TUNNEL CONVEYOR. AS HE PICKED UP A LARGE ROCK OFF OF THE FLOOR TO PLACE IT BACK ONTO THE IDOL BELT HE STRAINED HIS L OWER BACK. HE HAD HAD A PREEXISTING BACK PROBLEM THAT ORIGINATED FROM A TRAFFIC ACCIDENT IN OCT 2001. LIFTING OF THE ROCK FURTHER AGGRAVATED HIS CONDITION. LOST TIME DID NOT OCCUR TILL 12/17
A MAINTENANCE CREW WAS WORKING UNDER THE PRIMARYCRUSHER FIXING A SUPPORT BEAM WHICH REQUIRED ATTENTION. EE WAS STANDING ON A HORIZONTAL BEAM 2' OFF THE FLOOR. AS HE STEPPED DOWN OFF THE BEAM HE SCRAPPED OR LACERATED HIS LEG ON THE BEAM.
WHILE CLOSING THE DOORS ON AN OPEN-TOP HOPPER RAILCAR HE STEPPED INTO AN ANIMAL HOLE CAUSING HIM TO TRIP, LOOSE HIS BALANCE AND FALL. IN AN ATTEMPT TO BREAK HIS FALL CAUSED HIM TO SPRAIN HIS S HOULDER ON IMPACT.
EE WAS WORKING IN THE SHOP ATTEMPTING TO SEPARATE A COUPLER FOR #59 LOCOMOTIVE WITH A HAMMER AND A CHISEL. NORMALLY THIS COUPLER SEPARATES QUITE EASILY, HOWEVER, HE HAD NOT REMOVED ALL OF THE BOLTS HOLDING IT TOGETHER. HE FIRST DROVE THE CHISEL INTO THE COUPLER WITHOUT SUCCESS, THEN HE PRIED ON IT, AND AS HE MOVED AROUND THE COUPLER CHISEL, UNDER PRESSURE SHOT OUT AND HIT HIM IN TH
WHILE PERFORMING HIS REGULAR DUTY OF INSPECTING RAIL CAR DOORS, EE USED AN UNORTHODOX AND UNSAFE PROCEDURE TO CONDUCT HIS TASK. HE STATED THAT WHILE WORKING IN THE DARK, HE HAD PLACED HIS FLAS H LIGHT IN HIS MOUTH TO HOLD IT WHILE HE INSPECTED THE CAR DOORS. HE THEN SAID HE SLIPPED AND FELL, CAUSING THE FLASHLIGHT TO HIT THE CAR, DAMAGING HIS TEETH AND INJURING HIS SHOULDER AS HE A
EMPLOYEE WAS HELPING TO EXTRACT A HAUL TRUCK THAT HAD GOTTEN STUCK IN THE MUD. HE INJURED HIMSELF WHEN HE LIFTED A CABLE, THAT WAS GOING TO BE USED, THE WRONG WAY.

EE WAS DESCENDING FROM A 35 TON HAUL TRUCK; AS HE BEGAN TO TURN HIMSELF AROUND TO GO DOWN THE STEPS, HIS FEET SLIPPED OUT FROM UNDERNEATH HIM. HOLDING ONTO THE RAILING CAUSED HIS BODY TO SLAM INTO THE STEPS (WHERE THE INJURY OCCURRED) AT WHICH POINT HE LET GO OF THE HAND RAIL AND FELL TO THE GROUND.
EE WAS SHOVELING SAND OUT OF THE BOX ABOVE THE ISC-82 CRUSHER. THE SAND WAS WET AND PACKED AND BECAUSE HE WAS REACHING OVER THE GUARD RAIL TO PERFORM THIS TASK HE PULLED A MUSCLE IN HIS FOREAR M. ALTHOUGH THE INCIDENT OCCURRED ON 4/19, THE COMPANY WAS NOT NOTIFIED THAT HE HAD OBTAINED MEDICAL TREATMENT UNTIL 5/2/.

WHILE PERFORMING HIS REGULAR DUTIES CLOSING DOORS ON AGGREGATE HOPPER CARS HE STRAINED HIS BACK. THE ACTION INVOLVED INCLOSING THE DOORS REQUIRES THAQT THE DOOR AND LOCK BE PRIED USING A 12# S TEEK BAR. THE PRYING ACTION CAUSED THE INJURY.
EMPLOYEE WAS PULLING THE LEVER TO OPEN S.57 STORAGE BIN WHEN THE GATE JAMMED, JERKING HIS BACK. ON 08/02/99, HE SWITCHED JOBS AND PRECLUDED THATTHE CONSTANT SITTING AND BOUNCING IN A NEW LOADE R IRRITATED HIS LOWER BACK AND CAUSED PAIN TO RUN DOWN HIS LEG. IN 1994, EMPLOYEE SUFFERED A SIMILIAR LUMBAR STRAIN WHILE DRIVING A TRUCK WHICH INDICATES A REPORTABLE PRE-EXISTING CONDITION.

HE WAS CLIMBING UP THE SIDE OF THE PRIMARY CRUSHER'S FEEDER WHEN HE LEANED HIS BODY AND FELL AGAINST A STEEL BEAM, WHICH IMPACTED HIS LOWER RIGHT RIBS. THIS OCCURRED 3-26-97. HE THEN HAD HIMSE LF CHECKED OUT AND RETURNED TO WORK WITH NO TIME LOST. AFTER SOME TIME THE SORENESS PERSISTED SO HE HAD HIS DR RE-EXAMINE HIM 5-5-97 TO DISCOVER HE HAD CONTRACTED PLEURISY AND WOULD MISS WORK

WHILE WORKING IN THE SHOP ON A GEAR THAT WAS LAYING ON BLOCKS, HE STEPPED OFF BACKWARDS ANDSLIPPED ON A PIECE OF METAL.
THE WATER RECOVERY TROUGH HAD BEEN BORKEN BY EE TRUCK CAB PROTECTOR. MAINTENANCE CUT THE PIECE FROM THE MILL. EE WAS DRAGGING THE TROUGH FROM UNDER THE MILL WHEN THE ANGLE IRON EE WAS PULLIN G BROKE LOOSE CAUSING HIM TO FALL BACKWARDS. EE PUT RIGHT ARM BACK TO BREAK FALL, BUT IMPACT JAMMED HIS SHOULDER, AS HE HIT THE GROUND.
EMPLOYEE WAS RIDING IN A COMPANY PICKUP TRUCK, SETTING IN THE BED OF THE TRUCK LEANING BACK AGAINST THE FENDER WELL OF THE TRUCK BED WITH HIS ARMS RESTING ON THE SIDE OF THE BED. THE TRUCK DR OVE OVER A BUMP WHICH CAUSED HIM TO BOUNCE UP AND DOWN OFF THE TRUCK BED. AS THIS HAPPENED HE HIT HIS BACK ON THE FENDER WELL THUS INJURING HIS BACK.

EMPLOYEE WAS ATTEMPTING TO LOOSEN A ROTOR ON A CRUSHER BY USING A PRYBAR. AS HE PRIED ON THE BAR IT SLIPPED. THE SUDDEN SLIP CAUSED HIM TO SHIFT HIS WEIGHT HIS WEIGHT ON HIS LEG ULTIMATELY BRE AKING HIS ANKLE. WHEN HIS ANKLE BROKE HE LOST HIS BALANCE, FELL BACK ONTO THE CONVEYOR HE WAS STANDING ON AND THEN TO THE GROUND.
EE WAS PUTTING BOLT SIN THE BACK OF #1 SCREEN IN THE MILL WHEN A GUST OF WIND BLEW DUST AND STEEL PARTICLES INTO HIS FACE. THE DUST BLEW AROUND HIS SAFETY GLASSES AND INTO HIS EYE.
EE WAS CUTTING TREES WITH A CHAIN SAW & CLEARING BRUSH AWAY FROM POWER LINES & CONVEYORS HE STATED THAT HE MUST HAVE BEEN USING MUSCLES THAT HE NORMALY DIDNT USE & AS A RESULT STRAINED HIS BAC K SOMETIME DURING THE DAYS
INJURED WAS RIDING ON A RAILCAR AS THEY WERE TAKING THEM OUT OF THE QUARRY YARD TO BE HOOKED ONTO OTHER LOADED CARS AND MADE READY TO SHIP. WHEN THEY APPROACHED THE STATIONARY CARS HE STEPPED OFF OF THE RAILCAR HE WAS ON TO SIGNAL THE LOCOMOTIVE OPERATOR TO STOP IN ORDER TO CONNECT THE CARS. HE STEPPED DOWN HE SPRAINED HIS KNEE. ALTHOUGH THE INJURY OCCURED ON THE 8TH
DUE TO FEDERAL SAFETY REQUIREMENTS WE WERE INSTRUCTED TO INSTALL GUARDRAILS AROUND THE CATWALKS ON ALL OF OUR LATE MODEL OFF ROAD TRUCKS WE USED MSHA APPROVED WEBBING TO ACHIEVE COMPLIANCE EE WHILE EXITING THE TRUCK GOT HIS FOOT CAUGHT IN THIS WEBBING CAUSING HIM TO FALL HE CAUGHT HIMSELF AS HE FELL CAUSING THE INJURY DIAGNOSIS WAS NOT PERFORMED UNTIL APRIL 14 1995

EE WAS WALKING DOWN THE STAIRS ON THE TELSMITH AND SEVERAL SMALL STONES WERE ON THEM WHEN HE SLIPPED ON ONE AND FELL. HE LANDED ON HIS BACK WITH THE EDGE OF A STEEL STEP JABBING HIM ON HIS LOW ER BACK. HE TOLD HIS SUPERVISOR THAT THE PAIN INHIS BACK INCREASED AFTER A FEW DAYS AND THAT IS WHEN HE NOTIFIED US THAT HE WAS GOING TO A DOCTOR.

UNFORTUNATELY, THE BAR DROVE THE SAFETY GLASSES INTO HIS EYEBROW CAUSING A LCAERATION ABOVE HIS EYE WHICH REQUIRED TWO STICHES.

EE WAS CLIMBING DOWN OFF OF A LOADER, AS HE STEPPED ON THE GROUND HE TWISTED HIS FOOT ON A SMALL ROCK. WHEN HE STEPPED ON THE ROCK HIS FOOT TWISTED UNDER HIM AND HE THOUGHT HE HEARD HIS ANKLE CRACK.
HE WAS IN THE PROCESS OF CLEANING AND CHECKING THE INSIDE OF SOME RAILROAD HOPPER-CARS FOR FOREIGN MATERIAL WHEN HE SLIPPED AND FELL. HE FELL DIRECTLY ON HIS ANKLE WHICH RESULTED IN A FRACTURE . THE OTHER RAILCREW MEMBERS GOT OUT OF THE HOPPERCAR THROUGH THE BOTTOM DOORS OF THE RAILCAR.
ON WEDNESDAY MORNING DURING A NORMAL PRODUCTION DAY, AN AGGREGATE BIN (100 TON) FELL FROM THE MILL STRUCTURE. WHILE THERE WERE EMPLOYEES WORKING CLOSE IN THE AREA, NO ONE WAS HURT OR INJURED. THE AGGREGATE BIN WAS PART OF THE ORIGINAL MILL STRUCTURE BUILT IN 1950. THIS BIN HAS NOT BEEN USED FOR THE PAST 10 YEARS. ALL OTHER ORIGINAL BINS BUT TWO HAVE BEEN REPLACED THROUGH THE YEARS.
INJURED HAS CONTRACTED CARPAL TUNNEL IN BOTH HANDS FROM YEARS OF WORK OPERATING TRUCKS AND OTHER HEAVY EQUIPMENT.THE SYMPTOM FIRST APPEARED IN FEBRUARY OF 1992 BUT WAS NOT DIAGNOSED UNTIL 06 1 992.

INJURED WAS UNDER THE TRUCK SCALE PLATFORM FLUSHING OUT A SUMP PUMP.HE CRAWLED UNDER THE MAIN BEAM WHEN THE TOP OF CONCRETE DECK CAUSED HIM TO INJURED HIS KNEE BY THE CONTINUOUS MOVEMENT OF HI S KNEE ON THE CONCRETE.
EE AND OTHER EMPLOYEE WERE ATTACHING A HOSE TO AN OVERHEAD AUGER WHEN SOME "AN" FELL IN HIS EYE CAUSING HIM TO LET GO OF THE HOSE. EE WASN'T PREPARED AS THE HOSE FELL STRICKING HIM AND CAUSING HIM TO FALL TO THE GROUND. HE PUT HIS RIGHT HAND OUT TO CATCH HIMSELF CAUSING A SPRAIN. BOTH EE AND OTHER EMPLOYEE ARE CONTRACT BLASTERS EMPLOYED BY TECHNICAL EXPLOSIVES CO.
WHILE WORKING IN THE MILL ON A SCREENING UNIT GRINDING OFF RUST AND SCALE WITH AN ELECTRIC GRINDER A SMALL PIECE OF STEEL BECAME AIRBORN AND HIT HIM IN THE RIGHT EYE.

SHOVEL OPERATOR WAS STEPPING UP ONTO SHOVEL TO START MACHINE FOR OPERATION. EITHER THERE WAS GREASE ON THE STEP OR ON HIS FOOT WHICH CAUSED HIM TO SLIP. HE WAS CARRYING A THERMOS AND LUNCH BOX WHICH PREVENTED HIM FROM NOT FALLING. HE FELL TO THE GROUND CAUSING STRAINS, CONTUSIONS AND A PINCHED NERVE.
