Employee Statement - Put tubes out. Had one remaining and took it to stemming pile where extra tubes are kept. Walking back to drill, I slipped on some stemming #6 stones that were on top of smooth bedrock. My right foot went up and my left foot ended up underneath me. I felt my left ankle pop.

EE was cleaning up after a shot and he didn't have his buckel all the way down on the ground, He ran over a rock. That made his upper body twist and his head hit the center post in the cab. He cut his head on the left side and had to get 2 staples in his head. I took him to hospital. He returned to work with out restrictions.

About and hour after accident he went to St. Lukes Hospital. They took x rays and was discharge )sprain right knee and ankle). He had wet shoes when he went to get into the truck.
EE ROLLED A ROLL OF WIRE ONTO ITS SIDE IN ORDER TO ROLL IT TO JOB TRAILER. EE FELT PAIN IN CHEST AND EXPERIENCED SHORTNESS OF BREATH. EMERGENCY PERSONNEL WERE CALLED AND TRANSPORTED TO HOSPITAL. IT WAS SUSPECTED THAT THE EE WAS SUFFERING A HEART ATTACK

WE WAS PULLING A PULLEY WITH A COMEALONG WHEN EEPICK UP THE OTHER SIDE OF THE PULLEY WITH HIS HAND, AND THE PULLEY WENT IN REAL FAST AND CRUSH HIS FINGER. 3 SUTURES WERE NEEDED
