Employee was doing plant repairs on 10/26/21. IE and co-worker were working together on different repairs all day. Co-worker was doing the welding, IE was helping cutting steel, grinding, and general labor. Later that evening IE had eye irritation and went to the ER. Doctor took IE off work 10/27 and 10/28.

Miner tripped and fell, striking head on a handrail.

EE was cleaning up the haul roads and opened an existing weep hole in the berm. When EE opened the weep hole, hit a 16"" natural gas line. When EE hit the gas line it stalled the engine on the dozer, and the pressure broke the glass out of the machine. The pressure was also throwing sand and gravel into the cab of the machine.
The miner was installing flight on a screw. EE was turning the screw by hand, when they pinched their finger between the v-belts and the pulley. Left index finger was cut and smashed.

A company mechanic was replacing the starter on a Cat 730 articulated haul truck. When he removed the bolts from the belly pan it fell smashed his left index finger. He had a laceration about 3 inches long on his finger. He was taken to urgent care where he received 9 stiches.
Miner was climbing down a ladder and twisted his knee on dismount.
