Struck against stationary object

Employee was pulling the chain to open the front gate when he was swing his right arm up to get the slack out of the chain his right wrist hit the corner of the office hour sign. This caused a laceration.

He was operating the clam dredge. He was adjusting a water hose, when bucket came back up and flung a rock at him. The rock struck him in the L upper arm, causing a laceration.

EMPLOYEE WAS WALKING UP STEPS TO STORAGE TRAILER AT MAIN PLANT. GOING FROM 1ST TO THE 2ND STEP HIS FOOT SLIPPED OFF 1ST STEP CAUSING HIM TO FALL FORWARD LANDING ON HIS RIGHT ARM & HAND RESULTI NG INJURY A STRESS FRACTURE OF THE BONE BETWEEN INDEX KNUCKLE AND THUMB OF RIGHT HAND. DR PLACED A REMOVABLE SPLINT ON RIGHT HAND.

EE WAS INSPECTING THE RADIAL STACKER THAT HAD BEEN LOWERED TO THE GROUND STANDING ON THE UPPERSUPPORT LEG OF THE STACKER EE JUMPED OFF LANDINGON A ROCK ON THE GROUND CAUSING HIS ANKLE TO TWIST OVER BREAKING THE SMALL BONE IN HIS LOWER RT. LEG. APPROX. A 4' JUMP.

EE WAS HOISTING A GEAR ON A CRUSHER BEING REBUILT IN OUR SHOP. THE GEAR SHIFTED OFF THE CHAIN USED FOR LIFTING, PINCHING HIS WRIST BETWEEN GEAR AND SHOP FLOOR.

EMPLOYEE WAS STORING SCREENS IN STORAGE TRAILER AT MAIN PLANT. EMPLOYEE ATTEMPTED TO MOVE A SMALL PALLET THAT SCREENS WERE BANDED TO FOR SHIPPING. EMPLOYEE DID NOT NOTICE PALLET WAS BANDED TO ANOTHER PALLET FULL OF SCREENS UPON TRYING TOLIFT PALLET HE STRAINED HIS BACK.

EMPLOYEE WAS WELDING AND GRINDING ON PROCESSING PLANT WHEN A PIECE OF STEEL SLAG BECAME LODGED ON RIGHT EYE. EMPLOYEE THOUGHT HE REMOVED THE SLAG AND CONTINUED TO WORK. THE EYE CONTINUED TO BE IRRITATED PROMPTING THE EMPLOYEE TO SEEK MEDICAL HELP IN COMPLETE REMOVAL OF SLAG. TWO FORMS OF EYE PROTECTION WAS WORN.

OPERATOR WAS HELPING THE MECHANIC BEND A PIECE OF ROUND BAR USED TO MAKE A CABLE GUARD FOR THE DRAGLINE BOOM. A FLAKE OF RUST CAME OFF ROUND BAR LODGING IN OPERATOR'S LEFT EYE.
