EE WAS ATEMPTING TO REMOVE A DETACHABLE BUCKET FROM SKID STEER LOADER. THE BUCKET WAS RAISED SEVERAL INCHES OFF GROUND. EE HAD RELEASE LATCHES OPEN ON BUCKET, BUT BUCKET WOULD NOT RELEASE DUE TO MATERIAL BUILT UP ON LATCHES. BUCKET THEN RELEASED AND FELL ON EE'S LEFT FOOT.

EE WAS CLEANING OUT THE CHUTE THAT HAD FROZEN MATERIAL ON THE CONVEYOR BELT AND WHEN HE WENT TO KNOCK IT OFF HE GOT HIS FINGERS CAUGHT BETWEEN AN ANGLE IRON AND THE FROZEN MATERIAL.
