Maintenance employee was working on an aggregate discharge chute under a non-operating screening unit, from a step ladder. Weather conditions were damp, light rain. Employee slipped from ladder step while reaching for a part, falling approx. 6-7 ft, landing on a conv. tailpulley guard on his left side, resulting in a mild concussion and 4 broken ribs.

UNDER FOOT WAS ICY AND SLIPPERY, EE HAD LOADED TORCH CART INTO THE BUCKET OF THE LOADER. EE WAS ROLLING BUCKET BACK TO HOLD CART IN BUCKET, WHEN HE SLIPPED ON ICE AND GRABBED THE TOP EDGE OF T HE BUCKET TO KEEP FROM FALLING AS BUCKET WAS BEING ROLLED BACK AND HIS HAND SMASHED BETWEEN TOP OF BUCKET AND THE PIVOT ARM.
