Fall from machine

A fire started within the engine compartment of a CAT 390D Excavator. The fire moved to encompass the entire piece of equipment. While the Operator that was running the equipment was able to safely get away, EE was unable to use the steps on the equipment and had to jump off and landed awkwardly in wet material.

A fire started within the engine compartment of a CAT 390D Excavator; fire moved then moved to encompass entire piece of equipment. Operator that was running equipment was able to safely get away from the equipment. Equipment was a total loss.

The lid on the portable welder fell on EE's forehead and face. It resulted in a cut on the side of their nose which was glued by the physician.

Employee was removing cable rope from wire rope and it slid, pinching right hand between metal pipe and cable rope, resulting in a sprained hand and put on 3 days light duty. The hand was x-rayed and was found not to be broken.

Stepped up into service truck and something popped behind knee. I told him he need to go get checked & he said if it started to hurt him worst that he would go in the morning. Called in on Tues 8/2/11 & said he would not be in to work today, because his knee was still hurting. I told him he need to call supt who told him to go to dr & report it on Work. Comp.

Crusher door was frozen shut.Trying to open it with a pry bar and the bar slipped and hit me in the head.

Welding in garage, opened garage door for ventilation, wind gusting 25 mph +, a big gust of wind blew helmet partially up and blew sparks up helmet into my eye.
