EE slipped on a patch of ice falling onto EE's right hand that was holding a hammer. EE fractured the middle, ring, and pinky fingers of right hand.
A roof fall occurred in M2 in by of the C33 Cross cut. This area was Barricaded at the time of the fall.
Roof fall occurred in L5/C84. The area had already been barricaded due to poor roof conditions. Following the fall the barricaded area was extended.
A roof fall occurred at a brow in L13/C74 intersection.
On the 19th of March the miner was part of a crew that stemmed 37 holes prior to a blast. The next morning, miner reported that miner's back hurt and the dispatcher recommended miner see a doctor. The miner went to the doctor but never reported the doctor's visit. Management found out about the restriction when they received a notification from workers compensation.

There was an unplanned roof fall above the anchor points in intersection C38 and L37.No equipment was damaged, No miners injured. There were no miners in the mine at this time. It was discovered at time of pre-shift inspection.
A roof sag was noticed in L27/C43. The area was barricaded off. The area fell on 7/20/21 and was found at approximately 10:50 am. by the day shift foreman.
At approximately 5:15 am the night shift foreman discovered a roof fall underground in L27/C41. The night shift Foreman called the mine manager and coned off the area. The mine manager called the MSHA hotline. The area remained coned off until an investigation could occur.
At approximately 11:55pm the night shift foreman discovered a roof fall underground in R1C14. The night shift foreman called mine manager and coned off the area. The mine manager called the MSHA hotline. The area remained coned off until an investigation could occur.
After arriving at the mine the night shift foreman started the inspection. During the inspection EE came across 3 roof falls on the right side of the mine in the coned off area. EE immediately exited the mine and called the supervisor. EE's supervisor then called the MSHA hotline. The area remained coned off until an investigation could occur.

A roof fall occurred in the C10 heading between headings M1 and L4. The area was not a working area as it had been barricaded and signed from travel a week earlier due to observed conditions. The fall was approximately 90 ft long, 20 ft wide and up to 15 ft above the roof line at its furthest points. It is estimated that over 500 tons of material fell.

Employee utilizing hammer to straighten bracket, assisting another employee. When the other employee hit the opposite side of bracket the energy caused the bracket to bounce up and hit employee across the lip. Causing laceration and broken tooth.

While reinstalling tail pulley guard employee was trying to line the guard with the frame. The guard shifted pinching his right small finger.
Employee was loading bundles of bolts weighing approx. 67 lbs. from a storage pallet to the loader bucket. Employee felt a pulling sensation in his back. He then visited a doctor who determined that he pulled a muscle in his lower back and placed him on light duty work
A roof fall above the anchorage occurred in the North6/West12 intersection. There were no injuries resulting from this fall. This area was inactive since 4/10/2015. The fall was approximately 40' wide by 40' long and 8 feet high. The fall area has been bermed off from travel.
A fall of ground occurred in the N8/W14 intersection. The area was approximately 50-55 foot long, 40 ft wide, and 6-8 ft high. The area had previously been bolted on approx. 6 ft centers with 6 ft long full column resin roof bolts. The fall was reported to the MSHA One Call hot line on 04/10/2015 at 8:57 A.M. and MSHA inspector investigated the area the same day.
Employee was operating a Gradall 4310 Mine Scaler in an underground room and pillar mine. Employee stopped the machine and shut it off. As the employee exited the machine he missed a step with his right foot causing him to slip and fall approximately 4 1/2 feet to the ground. When he hit the ground he twisted his left knee resulting in a soft tissue injury diagnosed as a sprain.

On 9-23-2014, a roof fall, above the anchorage zone occurred. This fall was located in the intersection of the west3 and north12 headings. The fall was approximately 50' wide, 80' long, and varied between 10'-15' above the roof line. This intersection was previously bermed off from entry, due to concerns of bad top in combination with the roof being saturated.

EE STATED HE WAS EXITING THE TRUCK TO GO TO THE STORAGE TRIALER AND SLIPPED AND FELL ON THE ICE. HIS ANKLE SLIPPED AND TWISTED AND HE FELT A SNAP.
