Fall onto or against objects

Loosening impellar on slurry booster pump when impellar came loose and caused me to slip and fall, hitting my lower back on the impellar housing.

Was operating forlift truck & was climbing on & off to help guide the 350 MCM electric cable that powers the dyna-jet. It started hurting early in the afternoon but at around 5:45 it started hurting so bad that I could not finish out my shift.

EE'S WERE HYDRAULIC MINING A FINE COAL RECOVERY POND WHEN A SLOPE FAILURE OCCURED CAUSING MATERIAL TO FALL TO THE WORKING AREA OF THE PIT. A STUDY WAS PERFORMED INDICATING FAILURE OCCURRED DUE TO OVERSTEEPING OF FACE DURING OPERATIONS. THE SLOPE MAY HAVE FAILED DUE TO COMBINATION OF HYDRAULIC MINING AND THE PROXIMITY OF AN EROSION CHANNEL ADJACENT TO THE SLOPE FAILURE.

Accident type, without injuries

Coal slurry highwall dislodged causing slide approx. 120' long 50' high 20' wide which covered mining equipment. Damage unknown at this time.

Fall to the walkway or working surface

Loose objects underfoot at job site (i.e. rocks); employee slipped on loose rocks and fell.

ROLLED AN ATV AFTER HITTING A DITCH. MY LEFT LOWER LEG WAS STRUCK BY THE ATV. LEG HAD SOME SWELLING. I CONTINUED TO WORK THE REMAINDER OF THE DAY.

ROCK GOT STUCK UNDERNEATH SLIDE DOOR FROM FEEDER HOPPER TO FEEDER THROAT. EE ATTEMPTED TO DISLODGE ROCK, USING 6' SPUD BAR. PLATE THAT EE WAS STANDING ON GAVE WAY, CAUSING HIS BODY TO DROP APPROX. 6"". EE LOST HIS BALANCE, THE SPUD BAR MISSED THE TARGET AND STRUCK THE DOOR, BOUNCING BACK AND STRIKING EE'S SAFETY GLASSES ON RIGHT SIDE OF FACE, CAUSING INJURY.

EE WAS HELPING LOAD A 55 GAL DRUM OF KEROSENE AND THE VALVE CAME LOOSE SPILLING KEROSENE ON HIS LEG. HE CONTINUED TO WORK WITH KEROSENE ON HIS PANTS LEG THE REMAINDER OF THE DAY CAUSING A CHEM ICAL BURN TO THE LEFT LEG. DID NOT SEEK MEDICAL TREATMENT UNTIL 8:00AM 12/7/2003.
