Was operating forlift truck & was climbing on & off to help guide the 350 MCM electric cable that powers the dyna-jet. It started hurting early in the afternoon but at around 5:45 it started hurting so bad that I could not finish out my shift.

EE'S WERE HYDRAULIC MINING A FINE COAL RECOVERY POND WHEN A SLOPE FAILURE OCCURED CAUSING MATERIAL TO FALL TO THE WORKING AREA OF THE PIT. A STUDY WAS PERFORMED INDICATING FAILURE OCCURRED DUE TO OVERSTEEPING OF FACE DURING OPERATIONS. THE SLOPE MAY HAVE FAILED DUE TO COMBINATION OF HYDRAULIC MINING AND THE PROXIMITY OF AN EROSION CHANNEL ADJACENT TO THE SLOPE FAILURE.

ROCK GOT STUCK UNDERNEATH SLIDE DOOR FROM FEEDER HOPPER TO FEEDER THROAT. EE ATTEMPTED TO DISLODGE ROCK, USING 6' SPUD BAR. PLATE THAT EE WAS STANDING ON GAVE WAY, CAUSING HIS BODY TO DROP APPROX. 6". EE LOST HIS BALANCE, THE SPUD BAR MISSED THE TARGET AND STRUCK THE DOOR, BOUNCING BACK AND STRIKING EE'S SAFETY GLASSES ON RIGHT SIDE OF FACE, CAUSING INJURY.
