EE'S FOOT SLIPPED ON ICE AS HE STARTED TO CLIMB INTO HIS LOADER AND HE PULLED HIS LEFT SHOULDER.

EE WAS CLIMBING DOWN THE LADDER OF THE PRIMARY JAW, SLIPPED, AND TWISTED HIS LOWER BACK.

EMPLOYEE SLIPPED AND FELL WHILE WALKING DOWN A STEEP SLOPE.

EE WAS SHOVELING UNDER THE TAIL PULLEY ON BELT &PULLED MUSCLE IN LEFT SHOULDER.
