Changing bucket teeth on a Caterpillar 374 excavator. Using a sledgehammer to remove the teeth, a 7mm chunk of tooth broke off and entered the operators left leg just above the knee.

Employee was splicing a conveyor belt (#1A) when upon repositioning themselves they unhooked their safety harness and fell before reattaching to conveyor frame. On 5-18-16 employee was lifting an auger and twisting around, and felt pain in back. EE declined medical treatment and remained at work. On 6-6-16 EE lifted something and aggravated lower back. EE remained at work that day and went to the dr. the next day (6-7-16). EE is currently working on light duty.
