EE was removing the power modular from a dozer. He was standing between the frame of the dozer and the modular disconnecting hoses. He repositioned his leg and came in contact with an electrical plug bracket. He felt a pinch in his right leg but was unable to see what he was hitting with his leg. Wears a support wrap and did not see he had cut into leg until he changed clothes.

The E. was attempting to replace a trough roller structure on a conveyor. The E. placed the roller structure onto the conveyor frame and was attempting to put it into the upright position, between the belting and frame, when his thumb was "wedged" between the two. The E. experienced trama to his thumb and drove himself to the E.R..

Employee was fixing horns on a D-11 Dozer and left the cab to retrive tools from the ground. While walking down the dozer tracks the employee slipped and fell to the ground onto his right shoulder. He stated the area was justed cleaned and there was mud on his boots.

The employee was picking up material that had fallen off of a conveyor belt. Another rock came off of the belt and struck the employee on the right index finger at the first joint. The employee had a broken joint and required stitches.
While clearing material that became "jammed" from the discharge of the crusher, a rock fell onto the conveyor onto the employee's left ring finger. The rock caused a cut requiring stitches and broke the bone in his finger.
The employee was driving a haul truck and a fawn jumped out in front of him. The driver lost control and struck a berm and eventually went over it, falling 10-15 feet. The employee exited the truck under his own power and complained of neck pain. Seatbelt was worn.

WHILE PERFORMING ROUTINE MAINTENANCE, EE HAD GREASE INJECTED, FROM A PNUEMATIC GREASE LINE, INTO HIS LEFT THUMB. HE WAS TRANSPORTED TO BUTLER MEMORIAL HOSPITAL. HE HAD SURGERY ON HIS THUMB TO EXTRACT THE GREASE.

ON 8/20/02 THE EE WAS DRILLING AND A CHUNK OF MATERIAL FELL FROM A HIGHWALL (40 FT) THE HIGHWALL WAS BENCHED 30 FT. THE EE SEEN THE MATERIAL FALLING AND CHOSE TO EXIT HIS MACHINE. THE MATERIAL HIT THE FAR SIDE OF THE DRILL AS THE EE WAS EXITING. THE EE FELL TO THE GROUND AND WAS NOT ABLE TO PROTECT HIMSELF. THE EE WORKED THE SHIFT AND RETURNED NEXT DAY.
EE WAS BURNING METAL. AFTER CUTTING THROUGH THE PIECE, 1/2 OF IT SLIPPED FROM TABLE FALLING TO FLOOR. AS IT FELL IT PULLED THE HOSES OF THE WELDING TORCH CAUSING TORCH TO FALL. EE GRABBED AT T ORCH, BUMPING THE OXYGEN VALVE OPENING IT UP. THIS CAUSED AN INCREASED FLAME. THE NOZZLE OF THE TORCH WAS POINTED @ LT ARM CAUSING BURN. WENT TO ER, GIVEN MEDS & ANTIBIOTICS, RETURNED TO WORK
