Employee did not seek treatment until 02/25/2025. A pin for the excavator bucket was being knocked out and when it came out, instead of letting it fall, the employee held onto it, not realizing the weight. Employee's left ring finger was smashed in between the pin and the excavator bucket. The employee received a fractured finger and two sutures.

Employee was pulling the fuel hose from the fuel truck to EE's drill. EE slipped and fell onto EE's back and suffered a compression fracture.

According to employee's physician, this is not a work related injury or illness. The employee was operating a stone drill when they began to have shortness of breath. An ambulance was called and employee was taken to the hospital and admitted. Employee was diagnosed with a bleeding ulcer and fluid behind the lungs.

The employee was installing a transmission guard on a piece of Caterpillar equipment and caught employee's right foot between the guard and frame. Employee broke bone in top of foot. Employee did not seek medical attention until 3-5-2017.

Employee was cutting a conveyor belt with a utility knife and the knife slipped and cut left arm. EE went to a physician and they glued the laceration closed.

The employee was stepping from the walkway of the Komatsu WA800 wheel loader, onto a step to gain access in order to change the right tilt cylinder hose. He lost his balance when taking the step and fell 11' to the ground. He suffered a laceration above his right eye, a broken elbow and facial fractures.

Employee was exiting the skid loader when he fell, fracturing his wrist. After the accident, the foreman drove the injured employee home and he was then laid off. Allegheny Mineral did not find out about this incident until 1/24/14. Once Allegheny Mineral confirmed that this injury occurred and was not reported, the foreman and injured employee were both terminated on 1/27/14.
Stone was being lifted out of a bin using a crane and sling. The employee was placing the sling around the stone to be lifted. When the stone began to be lifted, it slipped out of the sling and rolled into the employee's left knee causing a patella fracture.
Employee was using cutting torches when a piece of hot metal got behind his goggles and into his eye. He went to the Emergency Room on 05/14/2013 and they removed the material from his eye and gave him a prescription for eye drops.

Employee was sorting bolts on his hands and knees. When trying to get up, he felt a sharp pop in his right knee. He was in severe pain and had trouble maneuvering. Employee also had a right knee injury in November of 2009 and had to undergo knee surgery.

While unloading an oxygen tank from a pick-up truck the worker felt a pain in his right hamstring. The worker went to a physician on 1/8/2009 and was diagnosed with a right hamstring strain. This injury did not result in any lost time or restricted duty.

While perfoming maintenance on our drill, the injured was working on a brake drum, it slipped and came down on right index finger and cut tip of finger off.
Loosening a filter when it quickly came loose the man strained right hand.

While pounding steel with a hammer, the man had a steel splinter in his right thumb which was removed at the emergency room.

While repairing the air line on a 100 ton rock truck a blockage came free and struck the man on the arm. Was treated at emergency room and released to work. The next day pain and redness occurred and he went back to the emergency room. He was in hospital 1 day and released back to work.
The injured man was helping a mechanic remove a water pump from his rock truck. The mechanic bumped the man on the lips with a wrench. A month later the injured man's dentist noticed that his crown post was loose. We were notified of the incident on 5/8/06.
While changing screens the employee felt pain in right elbow. She will receive therapy and was prescribed pain medication. No lost time.
2005 · 1 incident
The injured man claims he stumbled on a rock when leaving his car to report to work. There were no witnesses known at this time. He thought it was a sprained ankle. Worked his shift but did not report accident. Went to Emergency Room noon next day & was diagnosed with a broken left fibula. Was cast and he worked normal duty next shift.
