EE twisted left ankle stepping off a backhoe.

EE got debris in right eye while cleaning out an air filter.

Wind blew debris into EE's left eye.

Employee was replacing skirt rubber on discharge belt. Left thumb was in a pinch point. Employee was wearing gloves.

EE WAS WALKING AROUND PLANT SPRAYING BELTS TO DE-ICE. TRIPPED ON SHOULDER STRAP THAT WAS ATTACHED TO BOTTLE.

EMPLOYEE WAS CUTTING CONVEYER BELT WITH RAZOR KNIFE.

HELPING TO UNJAM CRUSHER, LEANING OVER RAILING PULLING ROCKS WITH HOOK, JAM BROKE LOOSE PULLED HOOK AND EMPLOYEE DOWN ONTO RAILING.

WALKING DOWN STEPS AFTER REMOVING A PIECE OF METAL FROM TUNNEL BELT. STEPPED OFF LAST STEP, CAME DOWN ON SIDE OF RIGHT ANKLE.
