Employee was working from the 2nd or 3rd rung of a ladder when it slid out and they fell off and landed on their side.

EE's left foot slipped causing left hand to slide down the siding they were removing.

EE dropped a rock onto their left, ring finger while trying to move it.

EE missed a step coming down the stairs and twisted left ankle.

Employee was placing a piece of flat 3/8 inch steel onto ground and pinched right hand pinky finger resulting in two stitches.

EE was welding and metal went into left eye

EE slipped getting off of drill cab and foot was caught on step and toe bent wrong way. EE did not go for treatment until June 17, 2018.

Lifting rock & dropped rock on left middle finger.

Cleaning millings out of truck and fell off of truck.

Employee was installing a return roller on the belt and it kicked back and struck him in the right eye.

Going up ladder to baghouse and felt pain in right knee.

Slipped off loader steps, spraining right ankle.

A piece of steel fell onto employee's knee while trying to move another piece.

While removing bolt from adjuster, gear box moved and pinched right hand

Employee was standing next to a pile of beams and while picking one up, another slid down on top of right foot.

Emp was clearing jam out of the crusher with a wedge on end of a chain when the crusher spit out chain causing wedge to drop jerking emp and rupturing capilaries in his left eye.

emp was screwing plywood to the wall when the screw kicked out and the bit struck knuckle.

EE WAS DISMANTLING PORTABLE CRUSHER, GETTING READY TO TRAVEL. LAYING DOWN FEEDER WALL. EE WENT INTO CRUSHER TO SECURE WALL. WALL FELL, PINNING BOTH ANKLES AGAINST FEEDER WALL.

Employee was standing on catwalk, reached over conveyer belt to remove rock. Picked up rock and twisted back.

EE WAS KNEELING ON TUNNEL BELT TRYING TO BREAK UP FROZEN CLUMPS OF STONE. BELT MOVED EE BANED FOREHEAD ON METAL CROSSBEAM.

GREASING ROLLERS WALKING UP CONVEYOR BELT GREASING, FOOT SLIPPED OFF SIDE OF BELT, HIT SHIN ON INSIDE EDGE OF ILER.

TRYING TO HOOK WATER HOSE UP ON CRUSHER, WHILE GOING DOWN LADDER TO SHUT OFF FEEDER, HE SLIPPED OFF LADDER AND FELL.

INSTALLING WARE PLATES ON EAGLE CRUSHER. PLATE FELL PINCHING RIGHT RING FINGER.

EMPLOYEE WAS COMING DOWN LADDER FROM SCREEN HOUSE, WAS IN A HURRY FOOT SLIPPED OFF LADDER RUNG AND FELL.
