Employee mechanic was cleaning a crusher and jammed EE's pinky finger resulting in a broken pinky finger

Employee reported to work on 6/11 an indicated EE had a hernia. Employee did not feel or report and stress or strain while working prior to this. Employee was sent to Workplace health for evaluation and it was determined the employee had a small Incisional Hernia. (Area of a prior surgery).

While removing bulldozer cutting edge, bolt sheered off and struck miner in the shin. This resulted in laceration and possible hairline fracture.

On the afternoon of 1/1/09 at approx 15:20 pm a civilian female person trespassed onto mine property and subsequently committed suicide within her personal vehicle while on mine property. Local emergency personnel were notified immediately.

While shoveling around conveyor, hard hat fell off miner. After picking up helmet and starting to stand, miner bumped the back of his head, in turn cutting head and requiring seven stitches.
