Employee reported to work on 6/11 an indicated EE had a hernia. Employee did not feel or report and stress or strain while working prior to this. Employee was sent to Workplace health for evaluation and it was determined the employee had a small Incisional Hernia. (Area of a prior surgery).

On the afternoon of 1/1/09 at approx 15:20 pm a civilian female person trespassed onto mine property and subsequently committed suicide within her personal vehicle while on mine property. Local emergency personnel were notified immediately.

ONE BLOW CAUSED A METAL FRAGME NT TO CHIP FROM THE CHAIN LINK AND BECOME AIRBORNE STRIKING SUBJ IN THE NOSE JUST ALONGSIDE SAFETY GLASSES AND BECAME EMBEDDED UNDER THE SKIN.
