Employee was attempting to ascend a ladder to the cab of a CAT 777 haul truck, when he slipped and fell on his back. Employee was in pain and tolerated it without reporting this original injury. Employee claims to have aggravated the injury on 6/11/09 while trying to bang a pin from a bucket of a CAT 365 excavator. The injury was not reported to the employer timely.

THE MECHANIC WHO WAS REPOSITIONING THE BOOM ATTACHEMENT WHILE EE WAS WORKING IN CLOSE PROXIMITY AND EE WAS BUMPED IN THE HEAD BY THE BOOM
EE WAS DRIVING A CAT 777 TRUCK WHEN HE HIT A HOLE OR BUMP IN THE ROAD. THIS CAUSED HIM TO BOUNCE UP & DOWN IN HIS SEAT WHICH JARRED HIS LOWER BACK.
