Employees were trying to lower the gates inside the rolling rock box. To lower the gate, 15 bolts needed to be removed. When the last bolt was removed, the gate slipped and landed on the foot of one of the employees.

Employee was conducting maintenance activities and was using a wrench and felt a pop in left wrist. Was seen at Occupational Medical Center where they were given a brace for their wrist.

Laborer while cutting out new skirting material out of old belting, their grip slipped causing laceration of left hand while using a box cutter.

The employee was tightening the nut on the head section of the 7'standard crusher. A suspended weight and flat bar were being used to conduct final tightening. The employee was on the other side of the head and they did not realize the tip of finger was in one of the bolt holes on the bar. When the bar was struck, the bolts broke and the bar shifted cutting the tip of the finger.

EE was standing behind the employee trying to remove a worn tooth on the loader bucket. A small piece of the tooth came off hitting EE in the left cheek area below the safety glasses.
