Employee got foot caught between a forklift fork and the side of a belt conveyor frame. Employee went to the hospital for care and the plant was notified of the injury. At this time, is is believed that the result is a broken ankle.
Employee reported discomfort in RT foot after attempting to load an empty chemical tote into the bucket of a rubber tire backhoe.

Employee was welding picking eyes onto a metal plate in the grizzly feeder/dump box when the metal plate slid onto employees left forearm. X-rays confirmed fracture and employee is set up to see an orthopedist. Employee received a cast for arm.
Employee was returning grease line after greasing equipment when the handle contacted employee's right ring finger. X-rays negative for fractures. Employee receive 4 stitches to the finger, antibiotics, and pain medication. Employee was referred to an Orthopedist. Released to restricted duty.

Employee was getting off dozer when they slipped off the tracks. The tracks were full of red clay. When they slipped the left leg buckled and fell on the track of the dozer. Pulled muscle on left leg.

EE jumped 3-4 feet to the ground, hurt right knee
Employee's had right hand resting on the side of the feed hopper near the top of it. EE was looking down into the crusher to see if the saddles were worn. As the feed hopper was being lifted EE did not remove their hand. This caused their fingers to get caught between the top of the feed hopper and the support beam the feeder slides back and forth on.

Employee was in the process of cutting blast hole tubes in half to tube the blast holes that EE had drilled. EE's knife slipped while cutting the tube and cut EE's leg above the knee.

EE was installing a rubber snout on the discharge hopper of C33 to C34. He was using a box knife to trim the rubber when the knife slipped and cut his lower left arm.
Employee was moving a 20 lb water pump for back-hoe to pass. When employee lifted the pump out of the way, his foot slipped in the mud causing him to twist his lower back.
