EE was told to tell crusher operator to turn off crusher so he could restart breaker. EE told operator to stop. Inj EE got on top of conveyor w/o locking in & telling anyone. They reset breaker & started conveyor without the EE getting off. The EE was carried 25' up the belt. EE jumped off and landed on his feet, hurting back & ribs.

EMPLOYEE WAS STANDING ON JAW CRUSHER WITH HAND ON HANDRAIL. THE HAMMER WAS WORKING ON A BIG ROCK IN THE JAW CRUSHER. THE HAMMER FLIPPED THE ROCK UP AND THE ROCK HIT HIS RIGHT HAND.
