Tech got into the back of the service truck to retrieve a hose. While stepping out of the truck, techs foot caught the tailgate of the truck bed and began to fall backwards. Techs hand slipped off the safety handle attached to the service bed and EE continued to fall backwards hitting the ground.

Working on primary crusher, removing belt clamp, belt clamp still under pressure, when the clamp came loose it pulled back knocking the ratchet from employee's right hand, wedging hand between belt clamp and support bar.
