The truck came to rest in an area of water spprox. 15 ft. deep. Employee was pronounced deceased on scene by medical emergency personnel.

THIS CAUSED THE BAR TO BE PULLED INTO THE ROLLER. EE HAD A TIGHT GRIP ON THE BAR & HIS FOREARM WAS CAUGHT IN BETWEEN THE BELT & THE ROLLER. EE WAS ABLE TO PULL HIS ARM FREE FROM THE BELT BY PULLING ON BICEP.
